Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: milk 45F, sliced american cheese 44F.
Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Note: Left it inside the refrigerator.
Equipment / Good Repair / Operation (corrected on site) Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Observed 1DR upright refrigerator at 43F. Per the manager it goes up because the oven is placed across the refrigerator.
Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. Note: Adjusted the thermostat.

Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. UNIT PULLED AWAY FROM WALL, DIRECTOR WILL HAVE THE FRONT WHEELS LOCKED, SO THE UNIT WILL NOT SLIDE BACK TO THE WALL, MILK THROWN OUT AND UNIT WILL BE SERVICED A.S.A.P.
Certified Food Manager/Certificate Process Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc. Director (PIC) stated that the regular mgr. has an updated card, because she signed the check for it and it is not on-site.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card will result in further enforcement action.

The purpose of this visit was to conduct a routine inspection. The rest room near kitchen window was removed and the VA Uniform State Wide building Code requires a working exhaust fan in rest rooms without windows (and in since 2006 is required in all NEW rest rooms, window or not). Installing an exhaust fan is recommended. All problem items from previous inspection have been corrected. Thank you. Please monitor water temperature to keep it 110F and over. Quat test showed 300 - 400 ppm, Ok. Facility Data Sheet Hot water heater: AO Smith BT-80-230 which uses 75,000 BTU's to produce 82 GPH of 120F water at an 80F rise. Dishmachine: N/A No violation noted during this evaluation.
