The purpose of today's visit was to conduct a routine inspection. Observed good employee hand washing. A very good date marking system is in place. Facility was clean and organized.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.

The purpose of today's inspection was to conduct a risk factor assessment. Person in charge demonstrated good food safety knowledge. Observed a very good date marking system. It is recommended that foods that are prepared to be placed along prep tops be chilled to 41F before they are put in the prep top. Use the walk in refrigerator or refreeze to help with the cooling. The next inspection will be in approximately 6 months.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: pico de gallo 49F, sour cream 50F, milk 44F
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. CFM removed pico de gallo and sour cream and placed in walk in to cool. Milk was removed from prep top and placed underneath in the refrigerator.

The purpose of this visit is to conduct a complaint inspection. The manager was aware of the complaint and stated the customer called back to state that he actually had the flu and it was not from the food. There are several systems to track food safety within the establishment such as daily temperature logs, cooling logs, date marking and third party audits. There was a consumer advisory in place for those items that may be cooked to order. The burger is fully cooked to 155F even though it has a consumer advisory. During the inspection there were no critical violations observed.

  • Critical: Air Gap Design Standard, Not Less than 1'
    Observation: An air gap between the water supply outlet and the flood rim level of the hand held spray at the three compartment sink is not observed. Spray falls into the sink and there is not al least an inch of an air gap.
    Correction: An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch.

The purpose of today's visit was to conduct a risk factor assessment inspection.
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.
09/18/2014Risk Factor
The purpose of today's visit was to conduct a complaint investigation in conjunction with a routine inspection. The Health Department received a call from the Fire Department after they responded to a fire alarm at the above facility this morning. Per the Fire Department no fire occurred however the hood system did activate/discharge. Upon arrival EHS observed that the facility had voluntary closed for business. EHS observed that a third party fire line company was on site. The third party was on site to determine why the hood system discharged. The company determined that there was a leak in the pressure line which runs behind the hood filters and was on site fixing the leak. Once the repairs are complete, the facility plans to re-open. The facility drained out all cooking oil under the hood system and removed all food from the 8 drawers cooler under the hood system. All food contact surfaces were removed from under the hood system and were washed, rinsed and sanitized. No violations were observed during the routine inspection. The facility is approved to re-open after the repairs are complete.

* Please see attached certified food manager card information and fax or email a copy of CFM card to Fairfax County Health Department.
* Please fax or email verification of implementation of Employee Health Policy to Fairfax Coiunty Health Department.
* All critical violations must be corrected as soon as possible and no later than prior to next inspection.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

(see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action. Discussed with manager.
