--Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the (bar) low temperature sanitizing dishwasher was 0 ppm and not 50-100 ppm as required. The chemical jug was empty--a new jug was attained & at last check the chlorine level was 100 ppm. THIS WAS CITED PREVIOUSLY >24 MOS AGO. MONITOR DAILY TO ENSURE COMPLIANCE.

--Observed soiled linens (apron and rag) stored on the clean rack in the dish room which may contaminate food utensils or equipment. All removed and corrected. PREVIOUSLY CITED.

--BOTH outside doors (upstairs and downstairs) have gaps present and do not protect against the entry of insects, rodents, and other animals.

--The hot water heater to the bar 3-bowl sink was not producing enough hot water. Water probed at 98*F. Unit was turned up and was 111*F at last check.
