--Food employee was observed in prep area, not wearing proper hair restraint, such as a net or hat.

--Utensils being used in contact with food without being properly cleaned and sanitized prior to use, as evidenced by observing food handler wipe prep knife on his clothing after making a sandwich. (I immediately put the knife in the sink and advised the mgr.)
Food Storage Requirements & Prohibited Areas (46.321)
--Observed bag of onions stored directly on the floor in walk in cooler, rather than 6 inches off of the floor as required.
Preventing Contamination from Food Employees' Hands (46.261)
--A food employee was observed touching bread - a ready to eat food - with bare hands.
Preventing Contamination from Ice Used as a Coolant (46.286)
--Bag soda rack was stored directly below drain lines in basement as evidenced by the presence of waste residue on one carton. The residue came from a drain that was showing signs of leakage near the ceiling next to the beer cooler.
Objective of Cleaning Food Contact Surfaces & Utensils (46.711(a))
--Stainless steel food containers were observed with food residue and were not clean to sight or touch when stored for use. (All affected pieces were removed and will be cleaned.)
