- KDA_5_20411A - --No handwashing sink in establishment. Manager stated they have been serving ice cream floats to consumers and hand dipping ice cream.
- KDA_6_501111A - --7 live small flying insects in wet mop head in back of establishment. Fly strip by mop storage area. Employee was unaware of pest control company/frequency.

- KDA_Defacto - --Establishment does not have its own bathroom, but is 20 feet from the mall restroom, which patrons and employees can use. --ice cream frozen --Water at sink in establishment was 91F. A handsink is required to be 100F. The temperature required for a 3 vat sink is 110F. An extra heater may be needed to get the water hot enough to comply with food code regulations.
