  • KDA_2_30114G - --In kitchen, employee was placing raw ground beef patties on the grill with gloved hands, then changed gloves and handled ready to eat buns without first washing hands. COS-Inspector, educated employee who then chose to wash hands and discard ready to eat bun. VD.
  • KDA_3_10111 - --In kitchen, gallon of milk in refrigerator with no date marking. COS-employee had no idea when milk was opened so she chose to discard VD,
  • KDA_3_50116A2 - --In pizza make table, shredded cheese 47, pizza sauce 49,cut lettuce 49 COS-employee said both shredded cheese and pizza sauce food items were placed in cooler less than 4 hours earlier so she placed the food items in freezer until ice could be added to deep pans and food items were placed back into cooler. Lettuce was placed in reach in cooler part of cooler 1/10/2016 so employee chose to discard VD. Ambient air temperature was 43. General manager said the procedure is to place food items on ice when in cooler top make table.
  • KDA_3_50118A2 - --In kitchen, walk in cooler, milk, not date marked. COS-Employee said milk was opened 1/9/2015 but decided to discard VD. In kitchen, reach in refrigerator, sliced turkey and gallon of milk not date marked. COS-Employee said both food items were opened Saturday 1/9/2015 so she chose to date mark.
