KDA_3_30211A4 - --In the back room, the following popped and ready to eat popcorn were in containers sitting on the floor with out coverings, cheese/caramel, cinnamon/cheese/caramel, cheese. COS-popcorn was covered. KDA_3_30412B - --In the back room, a container of ready to eat cheese popcorn had two scoops in it with the handles touching the popcorn. COS-scoops were removed. KDA_3_30511A1 - --In the back room, there was cheese/caramel, cinnamon/cheese/caramel, cheese and KC mix sitting on the floor without lids. COS-lids were placed on the popcorn. KDA_3_50118A1 - --In the Kenmore reach in, an open gallon of milk was with out a date mark. Person in charge said it was open on 12/3/15. Item was held over seven days. COS-(Corrected on site) milk was voluntarily discarded. KDA_4_60212B - --The microwave is lightly soiled on the interior. Person in charge said it gets cleaned every other week, unless they spill something. KDA_4_90311B - --Located on the make table in a white pull out stack drawer, single unwrapped spoons were placed where they the lip contact surface would be touched. KDA_5_20511A - --The hand washing sink had a bowl in the basin, preventing hand washing. COS-(corrected on site), bowl was removed. KDA_6_50111 - --There is base trim missing through out the back room. KDA_6_50112A - --Wall above pop carbonator, has brown debris on it, also there is debris on the wall above the hot water heater. The vent covers on the reach in cooler and reach in freezer has dust debris. KDA_7_10211 - --In the front area, there were two bottle located on a shelf with out a label. One had a green liquid, person in charge said it was 409. The other bottle had a clear liquid, person in charge said it was 50% rubbing alcohol and 50% water. They use it to clean the popper.
