KDA_2_40111 - --The kitchen employee was chewing gum during food preparation. KDA_3_10111 - --Located in the walk in cooler (WIC) there was a pan of twelve stuffed peppers without a date mark. The kitchen employee said that he was not sure when the peppers were made. Corrected on site (COS)- discarded. --There were approximately 70 individual packages of reduced oxygen packaged tilapia thawing in the three vat sink. The packages had not been opened. The kitchen employee indicated that these packages arrived at 10:30 yesterday morning and he had placed them in the WIC to thaw. COS- cut open the packages and moved to WIC. KDA_3_30211A1b - --Located in the WIC, there was a box and a container of raw shell eggs stored in contact with two cartons of pasteurized egg liquid. COS- separated. KDA_3_30212 - --Located on a shelf above the food prep counter, there was an unlabeled container of brown powder which the kitchen employee identified as yeast. Located in the kitchen storage room, there were two unlabeled containers of white powder and one unlabeled container of brown powder which the kitchen employee identified as powdered sugar and brown sugar, respectively. Located in the kitchen storage room, there was an unlabeled bin of light brown powder which the kitchen employee identified as wheat flour. Located in a drawer under the food prep counter, there was one scoop that was pitted and had paint chipping. COS- discarded. KDA_4_20216 - --There was a wooden knife block with two knives stored in it located on a shelf above the food prep sink. The knife block is not constructed to facilitate easy cleaning. KDA_4_50111A - --Located on a counter next to the WIF, there was a slicer that had a broken handle. The laminate on the shelving under the steam table counter is coming apart. KDA_4_60111A - --Located on a shelf under the steam table counter, there was one muffin pan stored as clean that was soiled with food debris. COS- moved to wash area. Located on the knife strip above the food prep sink, there were two knives stored as clean that were soiled with dried food debris. COS- moved to wash area. Located in a drawer at the food prep counter, there was a scoop and a spatula stored as clean that were soiled with dried food debris. COS- moved to wash area. Located in the storage room at the back of the kitchen, there were two plastic food containers stored as clean that were soiled with dried food debris. COS- moved to wash area.
