The plastic wrap machine has an accumulation of black sticky residue on the exterior. The table holding the mixer has an accumulation of dried food debris. The shelving in the WIC has an accumulation of sticky debris. The back splash of the soda machine has an accumulation of sticky residue. The step ladders have an accumulation of black debris.
KDA_4_60211E4 - --The ice machine guard plate had slimy brown mold on the edges. COS. PIC cleaned item.
KDA_4_90312A - --Located in the cabinet under the soda machine, there were several sleeves of single use cups and lids being stored under exposed drainage lines. COS. Items were moved.
KDA_5_20212A - --The hand sink in the kitchen did not have any hot water. PIC said the hot side was turned off because the faucet leaks. COS. Water was turned back on. Educated staff.
KDA_5_20515B - --The hand sink in the kitchen area has a constant leak from the faucet. The AVB on the mop sink leaks when the water is turned on.
KDA_5_50117 - --There is no cover on the trash can in the ladies toilet room.
KDA_6_50111 - --The ceiling tiles in the eating area have brown stains.
KDA_6_50112A - --The ceiling vents in the eating are have an accumulation of dried lent and grease debris. The light fixtures in the eating area have an accumulation of dried lent debris. The back door and floor area by back door have an accumulation of sticky black residue. The wall by the hand sink and walls by the 3 compartment sink have an accumulation of black food splatters. The floors under the 3 compartment sink, inside the WIC and in the storage room with the hot water tank, all have an accumulation of black debris and trash debris. The light switch plates in storage rooms and in the kitchen have an accumulation of black debris.
KDA_7_10211 - --Located on the shelving under the mixer, there was a spray bottle of clear liquid with no label. PIC said it was bleach water. COS. Bottle was labeled.
