It was tempted around 1:45pm and the cook manager said the make table was restocked about an hour ago. The ambient air temperature of the unit is reading 35.4F. COS-kept the lid closed.
  • KDA_4_60111A - --On the utensil rack there were 2 metal food grade containers with a sticker andfood debris left on the outside of the containers. COS-moved to the ware wash area. (Note: 2 out of 1000+ containers, less than 10%).
  • KDA_4_70211 - --The mechanical dish machine located at the front bar was in use, utilizes chlorine for sanitization and measured at 25ppm. The directions on the container reads 50-100ppm (do not drop below 50ppm). COS-the PIC primed the machine and it is now reading 100ppm. He also called for the machine to be serviced today at 2:30pm.

  • KDA_3_10111 - --In the downstairs storage area there were 2 bags of beets rotting. COS-discarded.
  • KDA_3_30211A1a - --In the WIC on a speed rack there was one tray of raw beef being stored above raw ready to eat cut potato fries. COS-rearranged items.
  • KDA_3_30211A1b - --In the WIC there was a plastic food grade container of cooked crab meat being stored directly next to a plastic food grade container of raw chicken and a plastic food grade container of raw salmon. COS-rearranged items. (Note: there was no leakage from the containers, the containers had lids on them).
  • KDA_3_30212 - --On top of the steam well drawers, there was a metal food grade container and a plastic food grade container with a white granular substances and was not labeled. The person in charge said it was sea salt. In the downstairs storage area, there was a large plastic food grade container with a white powder substance and was not labeled. The person in charge said it was powdered sugar.
  • KDA_3_50116A2 - --In make table #2 there was a plastic food grade container of butter at 48.1F. The lid to the make table was opened for lunch rush (in the unit for less than 4 hours). The ambient air temperature was reading 36.7F. COS-closed lid.
  • KDA_4_20211A2 - --On the clean rack in the ware wash area there was one rubber spatula with deep cracks on the food contact surface (not easy to clean). COS-discarded.
  • KDA_4_50111A - --The handle on the left side of the pizza make table is missing.
  • KDA_4_60111A - --On the utensil rack there was three (22 qt.) plastic food grade containers with stickers left on them and stored clean. There was two (1/6 pan) metal container, one (1/4 pan) metal container, one (1/6 pan) plastic food grade container with dried food debris on the inside and stored clean. There was two (4 qt) plastic food grade containers with stickers left on them and stored clean. There was one (4 qt) plastic food grade container with dried food debris on the inside and stored clean. COS-moved all items to the ware wash area. Under a metal prep table the large meat slicer had dried food debris on the blade. It was used today and stored as clean. COS-an employee cleaned it. On a shelf there was the large metal produce slicer/chopper that had dried food debris on the blades and stored clean. COS-moved to the ware wash area. The large mixer had dried food debris on the top metal plate where you insert the whisk and stored cleaned. COS-an employee cleaned it.
  • KDA_4_60211E4 - --There was mold on the metal frame of the ice machine where the lid closes.
  • KDA_4_70211 - --The mechanical dish machine located at the front bar was in use,utilizes chlorine for sanitization and measured at 25ppm. The directions on the container were 50-100ppm (do not drop below 50ppm). COS-the PIC primed the machine and now it is reading 100ppm.
  • KDA_4_90311B - --On the utensil rack there were approximately 5 plastic food grade containers stacked wet/clean, 3 metal food grade containers stacked wet/clean and 9 metal bowls stacked wet/clean. COS-moved to the ware wash area.
  • KDA_4_90411B - --In the back bar in a cabinet there was a plastic divider with clean utensils not stored in a way so that only the handles are touched by employees.
  • KDA_5_20212A - --The handsink located to the left of the pizza make table was reading 96.6F. The roof top handsinks were reading 95.5F. COS-all were adjusted and now reading 100F.
  • KDA_5_20515B - --The mechanical dish machine located at the back bar leaks water below the machine onto the floor. (Note: the person in charge called for it to be serviced).
  • KDA_6_30114 - --There were no handwashing signs at the handsinks located in the kitchen or in the restrooms. COS-left handwashing signs for the kitchen and other handwashing signs will be ordered for the restrooms.
  • KDA_7_10211 - --In the front bar, on the floor there was a working spray bottle with pink liquid and was not labeled. The person in charge said it was sanitizer. On the second level waitress nook there was 2 working spray bottles with yellow liquid and not labeled. The person in charge said it was a peroxide cleaner. COS-labeled all spray bottles.
