After helping customer, she went to the deli sandwich area and began to handle sliced ham without washing hands and changing gloves. COS Employee was educated.   • KDA_4_60111A - --Stainless steel pans under the MT in the produce area were stacked and stored as clean had dried food and food debris on the surface. COS The pan were removed and placed in the warewashing area to be washed, rinsed, and sanitized.
  • KDA_4_90111A - --Stainless steel food contact pans were stacked wet in produce prep area.
  • KDA_5_20511B - --18 plastic lids were stored on top of handwashing sink preventing employees to wash their hands. COS Lids were removed by employee. PIC was educated.
  • KDA_7_10111 - --A white bottle that had no visibility of the inside contents was stored under the 3 compartment sink. It did not have a label. Produce employee said it was bleach. COS Bottle was labeled.

  • KDA_2_30115 - --Salad bar employee rinsed hands in 3 vat sink after being instructed to wash hands before glove change after handling dish soap container. COS, instructed employee to wash hands only in hand washing sink.
  • KDA_3_10111 - --Box of assorted herbs in produce walk in cooler (WIC) with mold present on herbs. COS, moved to waste area
  • KDA_3_30412A - --Cups without handles stored in food in salad bar containers.
  • KDA_3_50118A2 - --Produce WIC: No date marking on container of feta cheese. Employee stated the feta was removed from original container 2 days prior. No date on container of cut ham, peas & imitation crab meat. Employee stated items were removed from original packaging yesterday at 9am, time of observation was 9:35am.

With the same gloves, she was then observed handle RTE food. COS Employee and PIC was educated. Employee washed hands and put on a new pair of gloves.
  • KDA_2_30114H - --Observed employee washing dishes in the 3 compartment sink. An order for some food came in, the then whipped his hands on his apron and proceeded to put on gloves. COS Employee was educated. Employee washed hands and put on a new pair of gloves.
  • KDA_2_40111 - --Observed an employee in the deli prep kitchen eating food over cooking equipment. COS Employee and PIC was educated.
  • KDA_3_10111 - --In the deli area, sliced turkey did not have a prepared or discard date. Employee was unsure when the meat was cut. COS Meat was discarded.
  • KDA_3_30211A1b - --In the deli WIC, raw shell eggs were stored on a wire shelf over RTE biscuits. COS Eggs were moved to bottom shelf.
  • KDA_3_30511A1 - --Near the deli exit door, a box of cups was stored on the steps floor.
  • KDA_3_30613B - --On the sales floor for consumer self service, dried cranberries had a large spoon was inside the container. The spoon handle was touching the cranberries. The RTE food was not protected if a customer was to provide shelf-service. COS PIC removed cranberries off sales floor. They will only be offered on the salad bar.
  • KDA_3_50116A1 - --In the deli hot holding display case, was bbq chicken nuggets 102F and fried chicken 107F had measured temperatures below 135F. Employee said the chicken was cooked 2:00pm. COS Chicken was put back in oven.
  • KDA_3_50116A2 - --Cut honey dew melon 47F, cut watermelon 51F, cut cantaloupe 50F, feta cheese 50F, blue cheese 51F, diced tomatoes 51F, bean sprouts 53F, alfslfa sprouts 52F, crab salad 51F, potato salad 52F, and pasta salad 46 had measured temperatures above 41F. Employee said the food was prepared at 6am. COS Food was discarded. When employee restocked food on salad bar, I instructed her to put the PHF on ice. In the deli display cooler, crab salad 47F, roast beef 50F, ham 47F, ham 46F, and rotisserie turkey 47 had measured temperatures above 41F. Employee said it was cut 8/10. COS deli meats were discarded.
  • KDA_3_50118A3 - --In the deli display cooler was baked potatoes, bowtie (cooked pasta) salad, mustard potato salad, baked beans, and loaded baked potato salad had the discard date of 8/18. Employee said the salads and baked beans were made today, 8/11. COS Dates were corrected.
  • KDA_4_30212A - --PIC and employee was unaware if a thermometer was available to check the temperatures of cold or hot holding food.
  • KDA_4_60111A - --In the deli prep kitchen, the can opener blade had dried food debris on the surface. COS Can open was clean during inspection.
  • KDA_4_60212B - --In the bakery, the microwave had dried food on the exterior and interior surfaces including the ceiling and walls.
  • KDA_6_20215A3 - --The dock door had 3 inch gaps at the bottom of the door.
  • KDA_6_50116 - --Wet mops in the bakery department, meat department, and the back storage area were stored in mop buckets to air dry. COS PIC turn the mop upright to air dry.
