- KDA_2_30114H - --An employee who was running the cash register, came to the and put on gloves with out washing his hands. He then dished up soup and then touched ready to eat hot pickles to put on plate. COS-gloves were changed and hands washed.
- KDA_3_30414B1 - --In the bar area, there were two wiping cloth on the cooling unit(that is not in use) that were wet. Employee said she uses them to wipe tables. Educated on them needing to be in sanitizer solution. COS-a bucket of chlorine sanitizer was made to 75PPM and cloths placed in there.
- KDA_4_10111A - --In the walk in cooler, there was a sterilite container that had sliced on site deli meat and cheese stored in it. Educated on food being stored in food grade safe containers. COS-food grade safe paper was placed in the container. Since these is less then 10% inspector issued FWL.
- KDA_4_60111C - --In the walk in cooler, there is black debris on the fan cover.
- KDA_6_50111 - --In the bar area, the flooring is concrete. There is missing flooring through out the kitchen and store area.
- KDA_7_20711B - --Sitting on top of deli case over where meat and cheese is wrapped, were four bottles of prescription medication. Person in charge said he just brings them in when he needs a refill. COS-bottle were discarded.
- KDA_Defacto - --Frozen food is frozen. --Hot holding on the stove, made on site chili 174.2F. --In the bar area at the three compartment sink, 100PPM chlorine sanitizer. --Walk in cooler, sliced deli ham 39.7F. Deli case left side, potato salad 38.8F. Deli Case right side, sliced deli ham 39.6F.
