KDA_3_30212 - --There was a container of sushi rice and a container of quinoa rice (identified by PIC) in containers in the sushi grill area without a common name listed on the label.
KDA_3_50116A2 - --There was a container of sliced tomatoes on a rolling tray by the cook line that measured at 53F. A cook was preparing the tomatoes at start of the inspection, approximately 1.5 hours prior. COS- the employee (he) moved the tomatoes to the walk in cooler for immediate cooling.
KDA_4_20211A2 - --There were 4-5 plastic food storage containers on a clean utensil shelf in the back prep area with large cracks and breaks along the food contact surfaces. COS- PIC voluntarily discarded.
KDA_4_60111A - --There were approximately 5-8 clean plastic food storage containers with old date stickers along the food contact surfaces stored on top of the line. COS- PIC moved the containers back to the warewashing area for additional cleaning.
KDA_5_20213 - --The spray hose located on the warewashing unit hangs below the flood rim level of the spray area where no air gap is being provided. COS- PIC hung the hose on the wall to provide an accurate air gap for backflow prevention.
KDA_6_20215A3 - --There is a 1/4 inch gap under the back exit door near the garbage unit that is large enough to allow the entry of pests.
KDA_6_501110B - --There were two employee purses on the shelf by the to go area next to a box of single service utensils.
KDA_7_20111A - --There was a wiping cloth QT bucket stored directly next to plastic food wrap on a shelf in the back prep area. COS- PIC moved the container to another area.
