KDA_3_50211C - --Jars of establishment made salsa were packaged in mason jar with the establishment made labeled on the jars. PIC stated he does not have a variance or HACCP plan for the establishment made salsa that was not refrigerated. Explained to the person in charge that product would have to kept below 41F, and date marked. COS, salsa discarded.
KDA_4_60111A - --Dried food debris was on the blade of knife that was stored as clean on knife magnetic wall strip. COS, knife removed for cleaning and sanitizing.
KDA_6_501111A - --4 fresh mouse droppings were on the floor behind a file cabinet in the back storage area. PIC provided pest control invoice with a service date 2-15-15.
