COS Spoon was removed from rack and was taken to the warewashing area to be washed, rinsed, and sanitized. In the warewashing room, stored as clean, 7 large food containers and plastic lids had food residue/crumbs and stickers on the exterior and interior surface. COS Containers were removed and placed in the warewashing area to be washed, rinsed, and sanitized. In the wait staff area, individual salsa bowls were stored as clean in a large container had dried food debris on the interior surface. COS Salsa bowls were removed and placed in the warewashing area to be washed, rinsed, and sanitized. In the kitchen, a onion cutter was stored as clean had dried food debris on the surface. COS The onion cutter was placed in the warewashing area to be washed, rinsed, and sanitized. KDA_4_90311B - --7 large containers were stored as clean and stacked wet. COS Containers were removed and placed in the warewashing area to be washed, rinsed, and sanitized. KDA_5_10311B - --Upon beginning the inspection, there was no hot water in the establishment. The hot water at the handwashing sinks, wait staff area and the bathroom had measured temperatures of 75F. Manager said the pilot light at the water heater was not on. COS Manager lit the pilot light. The water was hot. KDA_6_20211A - --The wait staff area, 3 spiral light bulbs that contained mercury were not shielded, coated, or was shatter resistant. KDA_6_501111A - --On the ledge of the stairs was a live roach. In the hallway to the bathroom were 2 drain flies and in the hallway to the warewashing area were 5 drain flies. In the basement, there were 6 fresh rodent droppings under the electrical breaker boxes. There were 3 fresh rodent droppings on the plywood in the water heater area. KDA_7_10211 - --On the kitchen floor was a red bucket that contained Quat sanitizer. It did not have a label.
