KDA_3_50119B4 - --In container on back side of prep line, a package of sliced queso cheese was marked "10/14/15, dicard by 10:35PM", exceeding the TPHC limit. COS - cheese discarded in trash. KDA_4_60111A - --2 Rubbermaid containers (1 holding canadian bacon, 1 at clean storage rack) had the residue of the original sticker of the container when purchased. COS - containers placed for cleaning. KDA_4_60111C - --The 2 side doors on walk in cooler had a build up of mold around the door seals. There is a build up of residue around in cup dispensers. There is a build up of dirt/debris under and around equipment in front waitress station. KDA_4_60211E4 - --The pop nozzles on self service machine (north end) have a build up of residue. KDA_5_20515B - --In cabinet underneath self service pop/ice machine, there are 2 buckets full of water. Manager stated lines are leaking and parts have been ordered for repair of the lines. KDA_Defacto - --Cold Holding: Reach in cooler (end of prep line) - canadian bacon 39F, breakfast burrito 41F. Reach in cooler #3 - 39.3F ambient. Undermount cooler (prep line) - 42F ambient. Walk in cooler - cut lettuce 40.2F. Small reach in cooler (drive thru) - yogurt 38.9F. Creamer cooler - 42.4F Small reach in cooler (front service area) - milk 35F. Freezers ok. --Dish machine sanitizer level tested at 100ppm chlorine. --Hot Holding: Hot Drawers - hamburger 149F, BBQ beef 172F, sausage patty 137F. --Reheating: Grill - RTE sausage patty - 180F. --Wiping cloth sanitizer: Bucket (by hand sink) - 100ppm chlorine.
