  • KDA_3_30211A1b - --There was a package of raw bacon stored directly above RTE pancakes and RTE folded eggs in the glass reach in cooler on the cook line. COS- PIC rearranged the shelves in the cooler.
  • KDA_4_60111A - --There were two plastic food storage containers on the clean utensil shelf with old date sticker residue along the sides. COS- PIC moved the containers back to the warewashing area for additional cleaning. There was old food debris build up on the food contact blade of the commercial opener. COS- PIC moved the opener back to the warewashing area for additional cleaning.
  • KDA_6_20215A3 - --There is a 1/4 inch gap at the lower right hand corner under the back exit door large enough to allow the entry of pests.
  • KDA_6_50111 - --There were several missing tiles at the front counter area closest to the window. PIC stated the tiles were loose so maintenance removed them and was unsure when the tiles will be replaced.
  • KDA_6_501112 - --There were approximately 5-6 dead cockroaches in the cabinets under the soda units in the lobby.
  • KDA_6_50112A - --There was dirt build up and sticky residue build up in the cabinets under the soda units in the lobby.

QT test strips provided. Chlorine measured at 50ppm in dishmachine. Chlorine test strips provided. --raw shell eggs 42 (glass reach in cooler on line) sliced tomatoes 42 (reach in cooler on line) liquid egg 40 (walk in cooler) creamer 43 (dispenser on counter) milk 40 (reach in cooler 1 front) yogurt 43 (reach in cooler 2 front) whipped cream 42 (cold hold on counter) butter 43 (glass reach in cooler front counter) --There is a pest control company contracted, no paperwork provided.
