KDA_3_10111 - --Located in the coffee machine in the bar area, there was a box of milk that was not date marked. Person in charge (PIC) said that he did not know when it had been opened. Corrected on site (COS)- discarded. Located at the bar, there was a bottle of Bacardi Gold rum that had a winged insect in it. COS- discarded. KDA_3_30211A1a - --Located in the make table at the back of the kitchen, there was a container of mahi fish stored over bags of cut lettuce. COS- moved fish. KDA_3_30211A1b - --Located in the walk in cooler, there was a box of raw beef stored over a bag of cooked pasta and a container with sauces that were cooling in an ice bath. COS- moved container of cooling sauces and package of pasta. KDA_3_50114A2 - --Located in the WIC, there were two containers of minestrone at 45F and 47F. Date mark sticker indicated that they had been cooked yesterday. PIC said that the soup had not been removed from the cooler since yesterday. COS- discarded. KDA_3_50118A1 - --Located in the RIC under the prep room MT, there was a container of hummus dated 8/20 - 8/26. COS- discarded. Located in WIC 1, there were packages of cooked rice noodles dated 8/25 - 8/30. COS- discarded. Located in WIC 1, there was a container of house made Italian dressing dated 8/27 - 9/2. COS- discarded. Located in the dessert RIC, there were two opened containers of cheesecake dated 8/25 - 8/31. COS- discarded. KDA_3_60311B - --The menu did not have asterisks next to foods that are available to be cooked to order (steaks). There was a disclosure statement at the bottom of the menu. KDA_4_20211A2 - --Located on the clean utensil storage rack, there was one rubber spatula that was chipped. COS- discarded. KDA_4_50111B - --Located in the wait area, there was a RIC that was cracked and falling apart inside. Located in the downstairs storage area, there was a chest freezer with a broken door. KDA_4_60111A - --80% of metal containers stored as clean on the clean dish storage rack in the back of the prep room were soiled with dried food debris. COS- moved to wash area. Located on a shelf under a prep counter in the prep room, there was a large pot stored as clean that was soiled with dried food debris. COS- moved to wash area. Located on a storage shelf in the wait area, there were 3 plastic pitcher that were soiled with sticker residue. COS- moved to wash area. Located in the wait station, there was a glass of iced tea spoons stored not inverted. KDA_6_30114 - --There are no hand washing signs in either of the men's or women's restrooms. KDA_6_50111 - --There are broken and missing floor tiles in the kitchen area behind the grill and in front of the far make table. KDA_7_10211 - --Located on the floor next to the mop sink, there were two buckets of blue liquid that were unlabeled. PIC indicated that they mix sanitizer in those buckets. COS- labeled. KDA_7_20111A - --There was a roll of paper towels stored in contact with containers of degreaser located on a shelf above the mop sink. PIC indicated that the paper towels are used for the hand washing sink. COS- moved paper towels.
