- KDA_3_50119B2 - --Establishment uses time as control on all items on the buffet line. Items cold held are also on time as control, however, there was no time stamp on anything on the cold hold buffet line. Items on the time as control written procedure include lettuce, tomato, and cantelope. COS PIC made time cards for the evening shift as there were none to place on the buffet line. (repeat from 7/7/15, and 7/22/15 inspection)
- KDA_3_50119B3 - --Establishment uses time as control on hot and cold held items on buffet line. The time stamp on the foods was from the morning, for example, chicken wings had a time stamp of 10:45AM, Dumplings had a time stamp of 10:45AM, Brocolli beef had a time stamp of 11AM, Seafood combination had a time stamp of 11:15AM. According to the time stamps, food held over 4 hours. PIC stated they had just been made for the dinner time. COS redated chicken wings etc to accurate times of 4:20-4:30PM initial time.
- KDA_4_60111A - --4 metal pans stored as clean on shelving across from dish machine with visible food debris on surface of pan. COS rewashed. In reach in cooler on cookline, plastic container holding breaded chicken with old date marking sticker residue on surface. (repeat from 7/7/15 and 7/22/15 inspections)
- KDA_4_70211 - --Employee washed bowl for handling food from make table top with soap water and rinsed with clean water before returning the bowl to the make table top unit to be used in food prep. Bowl was not sanitized before reuse.

- KDA_3_10111 - --Cooked chicken was stored on top of a raw shell eggs storage container that was inside a reach in cooler. Water a had dripped from the WIC cooler condenser unit and pooled on onto a crack lid that was stored on top of cooked chicken. COS, food items discarded.
- KDA_3_30212 - --A white crystal powder was stored in storage container without a common name on the working container. PIC, the white crystal like powder was sugar. A spray bottle with yellowish liquid was in side bottle was stored under a prep table. PIC stated the liquid in the spray bottle was cooking oil. COS, items labeled.
- KDA_3_30511A1 - --Containers batter, and containers of potatoes were stored on the floor of the food prep area.
- KDA_3_50116A2 - --Batter 79F made with raw eggs was stored in 5 gallon container on the floor. PIC stated the batter was made at 10
- KDA_4_20211A2 - --A plastic lid with cracks in the plastic was on top of cooked chicken in the walk in cooler. COS, chicken was discarded.
