KDA_3_50119B2 - --Single use cups filled with marinara are placed in the hot box from the walk in cooler and not heated up before hand. Marinara heats in the hot box until sold to consumer. Cups are marked with a date, but are not marked with a time. Temperatures of 2 cups were 61F and 156F. PIC stated some had just been placed in the hot box from the walk in cooler maybe 15 min ago, the others had been in the hot box since opening. PIC was unaware he had to put the time stamp on the marinara. Pizza boxes were marked with time.
KDA_4_20211A2 - --One plastic lid with cracks all the way through rim stored as clean on shelving above 3 vat sink for clean equipment.
KDA_4_60111A - --Stored as clean on shelving by 3 vat sink for clean equipment, 5 divided pizza pans with visible food debris on surface. COS rewashed.
KDA_4_90311B - --Metal pizza pans stacked wet on storage rack for clean equipment. Not allowed to air dry.
KDA_6_50112A - --Dirt buildup on floor under empty storage crates under first aid kit.
KDA_7_10211 - --Red bucket of clear liquid with rags in the liquid, on lower shelf by make table top with no label stating contents of bucket. PIC stated it was sanitizer, registered at 200ppm quat. COS labeled.
