  • KDA_3_30412F - --Above make table 1 there were two metal food grade containers with scoops sitting in water at 74.1F and 73.2F. The scoops are used for ice cream and sour cream. COS-dumped water.
  • KDA_3_50116A2 - --In the top right grill cooler drawer, there was a metal container of raw beef patties at 48.7F and 50.2F. In the top left grill cooler drawer there was a metal container of sliced ham at 44.6F. The drawers had been opened for lunch rush. The ambient air temperature of the unit was reading 35.8F. COS-kept drawers closed to help cool the products down.
  • KDA_3_50118A3 - --In the south bar reach in cooler there was an open gallon of milk date marked 10/29/15-11/5/15 (past 7 days). COS-date marked item correctly.
  • KDA_4_20211A2 - --On a shelf below a metal table there were 2 large plastic food grade lids with broken edges. COS-discarded. On the utensil rack above the mechanical dish machine there were four (2.4 qt) plastic food grade containers and three (1/3) plastic food grade containers with broken edges. COS-discarded.
  • KDA_4_30111 - --Make table 1 was not operating during the inspection. The establishment is using ice baths to help keep PHF products cold. (Note: the owner is aware and the part has already been ordered, the unit will be fixed on 10/31/15).
  • KDA_4_50111A - --The handles on make table's 2 and 3 are broken. The plastic hinge on the ice machine in the basement is broken.
  • KDA_4_60111A - --The meat slicer had dried food debris on the blade and stored clean. COS-cleaned blade.
  • KDA_4_90311B - --On the utensil rack (located in the basement) there were 18 metal sheet pans and 50+ metal food grade pans that were facing up. COS-inverted. On the utensil rack above the mechanical dish machine, there were 3 plastic food grade containers stacked wet/clean and not able to properly air dry.
  • KDA_5_20515B - --The spray nozzle at the garbage disposable sink leaks when water is supplied.
  • KDA_6_20211A - --The light fixture above the dry food storage area in the basement does not have a protective cover. The light fixture in the WIF does not have a protective cover.
  • KDA_6_20215A3 - --There is a 1" gap along the left side of the back exit door. This could allow the entrance for pests.
  • KDA_6_30112 - --The north bar did not have paper towels at the time of the inspection. COS-the PIC replaced some in the dispenser.
  • KDA_6_30114 - --There was no handwashing sign in the men's restroom. COS-left handwashing sticker.
  • KDA_6_50111 - --The wall below the 3 compartment sink and garbage disposable sink is deteriorating and exposed. There are approximately 20 broken/cracked floor tiles in the men's restroom. There are 2 broken floor tiles in the women's restroom.
  • KDA_6_501111A - --There are approximately 20 live small flies in the basement by the soda carbonator machine. (Note: they are staying contained in the area). There are approximately 5 live small flies by the dump sink at the south bar. There are approximately 5 live small flies by the handsink at the north bar.

  • KDA_4_20211A2 - --There was a food grade plastic container stored as clean that was burnt on the bottom container. COS Item was discarded.
  • KDA_4_60111A - --There were three food grade pans stored as clean on a shelf above the dishwasher, and three vat sink that had dried food debris on them from a previous use. COS items were placed in the dish room for cleaning.
  • KDA_4_60111C - --There is dried dust buildup on the fan covers located in the WIC-3.
  • KDA_4_60211E4 - --There is pink mold buildup on the inside ice machine deflector located in the basement.

  • KDA_6_30114 - --There is no hand washing signage posted at the hand sink located in the basement, and the mens and womens restrooms located outside. COS Hand washing signage was posted.
  • KDA_6_50111 - --There are several floor tiles missing in the mens and womens restrooms located in the establishment. There is sheetrock missing on the walls under the dish washer and three vat sink located in the kitchen area.
  • KDA_6_501112 - --there are 10-15 dead roaches, spiders, and box elder bugs on the floor in the basement area. Establishment has a pest control service. Last service was on 11/24/2015. Paperwork was provided.
  • KDA_6_50112A - --There is dried grease and food debris on the ceiling located in the kitchen area.
  • KDA_7_10211 - --There was a working spray bottle with a pink liquid being stored in the bar area that had no common name on the bottle. PIC said it was sanitizer. COS Chemical was marked.

In the basement walk in cooler there was a container of half and half at 51.4F, there was a plastic food grade container of cream of corn at 51.2F, packages of hard boiled eggs (shell off) at 52.2F. packages of avocados at 52.5F, packages of butter at 51.4F, a case of raw hard shell eggs at 52.0F, packages of cream cheese at 51.0F, half gallons of buttermilk at 51.3F, mozzarella cheese blocks at 51.4F, orange american cheese blocks at 51.5F and 52.0F, white american cheese blocks at 51.3F and 49.4F, sliced/blocks of cheddar cheese at 52.7F and 49.7F, block of provolone cheese at 53.2F, blocks of swiss cheese at 54.0F, bags of montereyjack shredded cheese at 51.3F and 50.8F, container of horseradish at 53.3F and 53.5F, a container of sour cream at 54.2F, cans of whipped light cream at 53.3F, a bag of shredded lettuce at 52.4F, sliced tomatoes at 46.5F, 44.4F and 52.1F, diced tomatoes at 51.3F, pepperjack cheese blocks at 48.4F and 51.1F, bag of parmesan cheese at 53.3F and bags of cut lettuce leaves at 54.8F, 54.4F and 52.9F.
