- KDA_3_10111 - --There was a fly inside a bottle of Canadian Club Whisky at the bar. Corrected on site (COS)- person in charge (PIC) moved whisky to office to take home for personal use.
- KDA_3_30212 - --There was an unlabeled spray bottle of clear liquid above the mop sink which the PIC identified as water. COS- labeled.
- KDA_4_50111B - --There is a piece of plastic coming off of the door to the Minute Maid RIC.
- KDA_4_60111C - --There is debris on the shelving of the WIC. There is debris on the inside of the Minute Maid RIC. There is an accumulation of grease on the outside of the fryer. There is an accumulation of grease inside the doors of the fryer. There is grease and debris on the outside of the kitchen grill. The vent hood is soiled with an accumulation of grease.
- KDA_5_20515B - --The left faucet at the three compartment sink leaks when the water is turned on. The faucet of the wash sink next to the white reach in freezer leaks when the water is turned on.
- KDA_6_50111 - --There are several cracked floor tiles in the WIC. There are several stained ceiling tiles in the kitchen.
- KDA_6_50112A - --There is debris on the floor next to the soda storage area next to the sushi station. There is debris on the wall under the sushi counter. There is debris on the floor of the WIC. There is debris on the wall and on the floor under the three compartment sink and the warewash machine. There is debris on the floor of the kitchen. There is debris on the floor behind the ice machine. There is an accumulation of dust on the vent cover at the back of the kitchen next to the storage room. There is debris on the floor of the storage room. There is an accumulation of debris on the shelf under the ice box in the server station.
- KDA_7_20212A2 - --There was a bottle of Raid Ant and Roach spray (EPA Reg. # 4822-447) and a box of Raid Fogger (EPA Reg. # 4822-452) in the kitchen. Neither of the pesticides had labels that said that they are approved for use in food establishments. The PIC said that he does his own pest control and that he sprays and uses the fogger in the kitchen. COS- educated on using approved pesticides/ licensed pest control operators. Moved pesticides to office to take home.
