- KDA_3_50115A - --There was a pan of chili, pork and potatoes being stored on the counter cooling with a internal temperature of 100.5F, pork rings, and chili with a internal temperature of 105.9F and no cooling method being used. PIC said it had been placed there about thirty minutes ago. COS Item was placed in the WIC.
- KDA_4_60111A - --There was a potato peeler and a metal spoon being stored as clean on a hook above the three vat sink with dried food debris on them from a previous use. COS Items were placed in the three vat sink for cleaning.
- KDA_5_20511B - --There were several plastic food grade lids being stored in the hand sink basin located in the kitchen area. COS Items were removed.
- KDA_6_30112 - --There are no paper towels at the hand sinks located in the kitchen area and in the front unisex customer restroom. COS Paper towels were placed at the hand sinks.
- KDA_Defacto - --Labels were made for the bagged chips, tostada, and churros the are made in the establishment. --RIC-1(frigidaire) cooked ground beef, hotdog, onion, peppers, and tomatoes 41F.
