- KDA_3_30611 - --Located at the sushi station, there was not any type of protection from contamination for the foods that the customers self serve. Foods were 5 plates of sushi, 1 bowl of wasabi and 1 bowl of ginger. COS. Plastic wrap was placed on top of food items. Educated staff to keep these items covered at all times and consider getting a permanent sneeze guard protector.
- KDA_4_50111B - --The door to the WIC is damaged, warped and broken. Metal pieces are sticking out of the door.
- KDA_4_60111C - --The doors to the WIC and door handles have an accumulation of black moldy sticky debris. COS. Doors were cleaned.
- KDA_5_20511B - --Located at the hand sink in the sushi area, there is a drain line running from the sushi display into the hand sink basin.
- KDA_5_20515B - --The faucet at the 3vat sink has a constant drip of water.
- KDA_6_50111 - --Located in various placed throughout the kitchen, there are cracks in the walls and floor molding.
- KDA_7_20111B - --Located on the shelving unit there were 2 cans of powder ajax cleaner sitting on the shelf directly above the open bag of flour and one of the cans was tipped sideways. COS. Chemicals were moved.
- KDA_8_30411A - --The current FE license could not be located. PIC does not know where it is. PIC will apply for a copy of the license.
- KDA_Defacto - --Grocery company delivering food during inspection. Chicken 41F --There was not any visible evidence of live or dead bugs. Areas of previous activity have been cleaned. Pest control company aggressively treated facility on October 13 and October 15.

- KDA_3_30212 - --Located in the WIC, there was no label on a container with pink food substance. PIC said it was crab meat.
- KDA_3_30412A - --Located in the WIC, there was a scoop laying inside a pan of chicken and the handle was laying in the food. COS. Scoop was removed.
- KDA_3_30412F - --Located at the self-serve sushi station, there were 2 tongs sitting inside a pan of water and the water temperature was 73.5F. COS. Tongs were removed, water was dumped, educated staff to keep tongs stored on a plate and wash tongs every 4 hours.
- KDA_3_30413 - --Located inside the sushi rice, there was a wet wiping cloth sitting directly on top of the rice. PIC said it was used to keep the rice moist. COS. Cloth was removed. Educated staff.
- KDA_3_30511A1 - --Located under the hand sink, here was a bucket of raw potatoes in water, sitting on the floor and it was sitting so that when you washed your hands, you dribbled into the bucket. COS. Item was moved. Located in the WIF, there was one case of salmon and one case of ice cream sitting on the floor. Located in the WIC, there was one bag of fresh carrots and one box of lettuce sitting on the floor. Located in the kitchen storage area, there was one case of dry noodles sitting on the floor.

- KDA_3_50119B2 - --Located at the sushi station, time was not being recorded for the sushi which was being held at temperatures above 41F. PIC said they use time as control. PIC said they make sushi and replace it if not eaten after 1 hour. Sushi was being held at a temperature of 72F, with no temperature control devices. PIC said sushi was made at 12:00 or 1:00. Current time was 1:30. COS. Time cards were placed at each sushi roll to record the time. Educated staff.

- KDA_4_60111A - --The table mounted can opener blade had dried food debris on the food contact surface. Employees did not know when it was last washed. COS. Item was placed in dish machine.
- KDA_4_60111C - --The exterior of the dish machine has an accumulation of dried food debris. The scoop used in the cornstarch had dried food debris on the handle. COS. Item was washed. The vent hood has a large accumulation of greasy debris. The kitchen sink, wall around the sink, soap dispenser, and towel dispenser all have dried food debris. The ice cream scoop holder has rust and moldy residue on the exterior. The RIF and RIC by the hand sink have an accumulation of dried food debris on the exterior and interior.
- KDA_4_90311A - --Located in the sushi area, there was one case of single use cups sitting directly on the floor. COS. Cups were moved.
- KDA_5_20511B - --Located at the hand sink in the dish area, there were 6 rubber gloves laying in the sink basin. PIC said they do not use the sink. COS. Gloves were moved. Educated staff. Located at the hand sink in the sushi area, there is a drain line running from the sushi display into the hand sink basin.
- KDA_5_501115 - --The dumpster area has an accumulation of trash and grease on the ground.
- KDA_6_20215A3 - --The front doors have gaps in the middle and bottom that are more than 1/4 inches.
- KDA_6_30511B - --Located in the sushi area, there was a personal coat laying on top of a case of RTE seaweed. COS. Coat was moved.
- KDA_6_50111 - --Located in various places throughout the kitchen, there are ceiling tiles missing, cracks in the walls and cracks in the floor molding. These areas have potential for insects and pests to enter the FE.
- KDA_6_501111A - --Locate in the hot water tank room, there were 5 live roaches crawling around. Pest control company comes twice a week. Last treatment was on September 30 and treatment was for german cockroaches. Located in the kitchen area, there were approximately 10 flies flying around the kitchen.
- KDA_6_501112 - --Located in the hot water tank room, there were approximately 20 dead roaches on the floor.
- KDA_6_50112A - --The floor areas in the kitchen have an accumulation of black debris along the base boards and under equipment. The floor areas in the hot water tank room and under the dish area have an accumulation of food debris, trash, dead bugs. The ceiling vents in the eating area have an accumulation of lent debris. The light switch covers in the kitchen area have an accumulation of black debris.
- KDA_7_10211 - --There was a large chemical sprayer device with tan liquid and no label. PIC said it was bleach cleaner. COS. Bottle was labeled.
- KDA_7_20111B - --Located in the storage cabinet by the cashier station, there were 3 bottles of window cleaner sitting on the shelf directly above 4 boxes of single use straws. COS. Chemicals were moved.
- KDA_8_30411A - --The current FE license was not hanging for the consumers to view. PIC did not know where it was.
