  • KDA_6_30112 - --There was not any hand drying supplies at the hand sink in the kitchen. PIC said they ran out last night. FE had been in operation for 3 hours before inspection had begun. COS. Single use towels were placed in dispenser.
  • KDA_6_50112A - --The floor area behind the fish tank has an accumulation of dirt debris. The floors in the kitchen have an accumulation of grease and food debris.
  • KDA_Defacto - --Kitchen hand sink hot water temperature was 107F.

  • KDA_2_40211A - --There were 4 employees working in the kitchen that had facial hair and no hair nets on their faces. Educated staff to wear facial hair restraints.
  • KDA_3_10111 - --Located in the RIC, one pan of raw fish in ROP, was thawing with no cuts in the packages. PIC said it had been thawing for less than one day (total) and it is still partially frozen COS. the packages were slit as an immediate corrective action.
  • KDA_3_30211A1a - --Located in the RIC, there were 2 pans of raw fish sitting on the shelf above one case of RTE raw cucumbers. COS. Fish was moved.
  • KDA_3_30211A1b - --Located in the WIC, there was a pan of raw lamb sitting on the shelf above a bucket of cooked RTE chicken. COS. PIC moved lamb.
  • KDA_3_30511A1 - --Located in the kitchen cooking area, there was one case of RTE raw potatoes and one case of RTE raw onions sitting on the floor. Located in the hallway storage area, there was one case of RTE raw potatoes sitting on the floor.
  • KDA_4_20211A2 - --Located on the shelving for clean dishes there was one small plastic container, out of 20, that was cracked. COS. Item was placed in trash can.
  • KDA_4_60111A - --Located on the rack for clean utensils, there was one cooking fork that had dried food debris. COS. Item was placed at dish machine. The table mounted can opener had dried food debris on the blade. Employees did not know when it was last washed. COS. Item was placed at dish machine.
  • KDA_4_60111C - --The RIC interior has an accumulation of dried food debris and food splatters. The exterior of the dish machine has an accumulation of crusty debris. The doors and door gaskets on the make tables have an accumulation of dried black food debris. The large mixer base has an accumulation of dried food debris.
  • KDA_4_60211E4 - --The ice machine guard plate had visible black mold on the edges. COS. Item was cleaned.
  • KDA_5_20511A - --The hand sink in the kitchen was not working and was covered with plastic wrap. PIC said the sink was broken and they were using the bar sink to wash their hands. Bar sink is on other side of the doorway and not convenient for the cooking staff. COS. Repair person was called and sink was repaired.
  • KDA_6_20211A - --Located in the kitchen area above the cooking stove, there was not a cover on the 4 exposed glass light bulbs. PIC said he ordered a cover.
  • KDA_6_30311A - --The WIC does not have adequate lighting. You have to use a flash light to see some of the food items.
  • KDA_6_50111 - --Located in the back hallway storage area the walls were peeling of paint.
  • KDA_6_50112A - --The floor area under the dish machine and other equipment in the kitchen and in the bar area have an accumulation of dried food and black sticky debris. The vent hood area and filters and lights have an accumulation of grease and lent debris. The light fixtures in the kitchen above the cooking area have an accumulation of grease and lent debris. The walls and light fixtures in the kitchen area have an accumulation of dried food debris and black debris. The back hallway storage area and the bar storage area has an accumulation of food debris, trash, shoes, and black sticky debris on the floors.
  • KDA_6_50116 - --Located in the kitchen area, there was one wet mop sitting on the floor and one wet mop sitting on the wringer. Neither mops were hanging to air dry.
  • KDA_7_10211 - --There was one spray bottle of clear liquid with no label. PIC said it was sanitizer. COS. Bottle was labeled.
