Raw Containers of Chicken stored above uncovered containers of sauce. No signs of contamination. PIC COS by moving the raw chicken. KDA_3_30212 - --Clear Plastic Spray Bottle under the hand washing sink in the kitchen contained a clear liquid. PIC indicated that it was water and COS by labeling. KDA_3_30412B - --Ice Bin Scoop was stored with the handle in contact with the ice. Cups were used for dispensing rice and other baking items. There was no defined handle and handle surface could not be determined. KDA_4_60111C - --RIC door had food debris accumulation and food debris on the bottom of the inside of the cooler. Kitchen hand washing sink faucet, towel dispenser and soap dispenser had accumulation of food debris. KDA_5_20511A - --Hand wash sink had hand washing reminder laying in the sink basin. PIC removed the reminder and COS. KDA_5_20515B - --Floor drain in the wait staff area was clogged and not draining. There was a puddle of water. KDA_6_20214 - --Employee Toilet Room Door was not self closing and open at the time of inspection. KDA_6_30114 - --Hand washing sinks in the kitchen, employee toilet room, and men's toilet room did not have hand washing reminders. Provided hand washing reminder stickers and COS. KDA_6_50119 - --Employee Toilet Room Door was not self closing and open at the time of inspection. KDA_7_10211 - --Clear Plastic Spray Bottle stored under a prep table contained a yellowish, green liquid and was not labeled. PIC said that it was degreaser and COS by labeling. KDA_Defacto - --No items were date marked. However, PIC indicated required items were cooked yesterday or this morning and had not been held more than twenty four hours. Educated PIC on the need to date mark items held past twenty four hours. --Rice cooked at 10:00AM was at a temperature of 66 at 11:30AM in the WIC. --WIF/RIF: Frozen.
