  • KDA_3_50116A2 - --On the food line sitting in a pan that was on ice, deli ham slices 44.6F. The pan had single serve containers stacked. Person in charge said it had been out for two hours. COS-they voluntarily discarded.
  • KDA_4_20211A2 - --On a shelf in the ware washing area, there was one lid that was cracked. On the make table in the ware washing area, there was one container that was cracked along the food contact surface area. COS-containers were discarded.
  • KDA_4_50111A - --There is an accumulation of ice in the south east corner of the walk in freezer.
  • KDA_4_50111B - --There is not a latch on the door of the walk in freezer.
  • KDA_4_60111A - --Stored as clean on a shelf in the ware washing area, four lids with sticker residue, two lids that had grease residue. In the biscuit making area, stored as clean on the make table, one container had sticker residue on it. COS-all items were placed for rewashing.
  • KDA_4_60111C - --In the walk in cooler, the shelf and floor on the west wall has food debris on it. There is a free standing fan by the ice machine that has an accumulation of dust.
  • KDA_4_90311B - --On a shelf in the ware washing area, there were three containers that were stacked wet. COS-containers were placed to be rewashed.
  • KDA_5_20515B - --The mop sink has two faucets that are leaking. The pipes along the north wall at the bottom of the stairs in the basement is leaking. water is running down the wall. There is a pipe in the middle of the floor that is leaking, causing water to pool on the floor.
  • KDA_6_30114 - --The hand washing sink in the ware washing area and the hand sink in the kitchen area, does not have hand washing signs. The hand washing sink in the kitchen area does have a glove use sign posted. COS- signs were left.
  • KDA_6_50111 - --The support beam by the three compartment sink has bare wood showing. There are several holes in the wall that lead down to the basement. There is a large gap under the walk in cooler door. In the walk in cooler, there is a piece of flooring missing, the south wall has a crack through it and the floor trim is loose.
  • KDA_6_50112A - --There is trash and food in the cabinets under the soda fountains. There is black mold on the inside and outside of the mop sink. There is dried on food debris on the wall behind the dish rack in the ware washing area. There is standing water on the basement floor in several locations. There is debris in the light covers in the register area. There is debris and stains in the light covers in the kitchen area. There is trash and debris behind the rack in the drive thru area. There is grease, dirt and debris located around the baseboard/floor area all through out the kitchen area. There is trash on the floor in the walk in freezer. There is black mold on the walls, floors, ceiling and door of the walk in cooler. There is mold and debris on the wall above and under the three compartment sink.
  • KDA_8_30411A - --The current license was located in the office on a bulletin board. Person in charge said he would move to a location where consumers would see it.
  • KDA_Defacto - --200 PPM Quaternary in the cloth wiping bucket. --Frozen food is frozen. --On the food line, sausage patty 151.9F. Under the warming light, tator rounds 143.4F. Under food warming light, wrapped chicken and biscuit 136.1F. By the grill in a steamer, sausage gravy 140F. --On the food line, yellow cheese slices 42.7F, deli ham slices 42.7F. In the Randell reach in top door, precooked hamburger patties 37.7F, bottom door, ham slices 37.7F. Walk in cooler, cooked peppers 36.4F. Bev-Air(E) in drive thru, ambient 38.5F. Egg wash on ice in the grill, 37.4F. --Tator rounds in the fryer, 204.8F. On the grill, scrambled eggs 166.4F.
