- KDA_3_30211A1b - --There were eighteen pans of par cooked(still raw bacon) being stored on a shelf directly above several packages of RTE cream cheese, soaps, and quiche located in the WIC. COS Raw bacon was moved to the bottom shelf.
- KDA_3_30412E - --There are two ice scoops being stored on top of the ice machine located next to the restrooms.
- KDA_3_50116A2 - --There was a pan of par cooked(still raw bacon)being stored on top of the steam table with a internal temperature of 97.7F. PIC said it was pulled from the WIC about thirty minutes ago. COS Raw bacon was placed back in the WIC. There was a bowls of cooked hash browns being stored on the rim of the grill that had a internal temperature of 73.7F. PIC said they were pulled from the WIC about forty five minutes ago. COS Hash browns were placed in a bowl of ice.
- KDA_3_50116B - --There was a pan of raw shelled eggs being stored in a plastic food grade container on the rim of the grill area that had a internal temperature of 55.3F PIC said they were pulled out from the RIC about ten minutes ago COS ice was added to the pan of raw shell eggs.
- KDA_3_50118A2 - --There was a container of commercially processed ham that was diced in the establishment stored on a shelf in the RIC-1(superior) with no use by date. PIC said it was prepared on 10/5/2015. COS Item date marked.
- KDA_4_20211A2 - --There was a plastic food grade cover being stored on top of a container of cut lettuce that has long deep crack on the lid located in the RIC-2(true). COS Item was discarded.
- KDA_4_60111A - --There were three food grade bin lids stored as clean with heavy amounts of dried food debris on them located in the back kitchen area. COS Items were cleaned. There was one soup ladle stored as clean in a drawer located in the back kitchen area that had dried food debris on it from a previous. COS Item was cleaned. There were two plastic food grade pan stored as clean on a rack in the back kitchen area that had dried food debris on them from a previous use. COS Item was cleaned.
- KDA_4_60111C - --There is dried dust buildup on the fan covers located in the WIC. There is dried food debris buildup on the inside shelf and out side doors of the RIC-1,2,MT, and the RIF. The shelf were the toasters are stored has dried food debris on them.

- KDA_6_30114 - --There was no hand washing signage posted at the two hand sink in the kitchen area and the one in the server area. COS Hand washing signage was posted.

- KDA_7_10211 - --There was a working spray bottle with a purple liquid in it being stored on top of the microwave located in the server area. PIC said it was degreaser. COS The bottle was marked.
- KDA_7_20111B - --There was a working spray bottle of degreaser being stored on top of the microwave located in the server area. COS Chemical was moved.
