COS PIC moved the RTE sliced chicken meat to the shelf above the raw sausage, and then took the raw sausage and placed it into a metal food grade pan.

- KDA_4_60111A - --There were seven plastic food grade container and on food grade plastic lid stored on a shelf above the MT with dried food debris on them from a previous use. COS Items were placed in the dish room area for cleaning. There was a one gallon plastic food grade jug the has sticky residue on the sides of the container from a previous use. COS Item was placed in the dish room for cleaning.
- KDA_4_60111C - --There is dried dust buildup on the fan covers located in the WIC. There is dried dust buildup on the hood filters above the dish washer. There is dried grease and dust buildup on the hoof filters and hood cavity located above the pizza oven.
- KDA_6_50111 - --The paint is peeling on the floor in the kitchen area.
- KDA_6_50112A - --There is dried dust, food, and grease on the walls and ceiling located in the kitchen area.
- KDA_7_10211 - --There was a working spray bottle with a clear liquid being stored on a shelf next to the HB with no common name on the bottle. PIC said it was QT sanitizer. COS Chemical was marked. There was a working spray bottle with a green liquid being stored on a shelf above the mop sink with no common name on the bottle. PIC said it was simply green. COS Chemical was marked. There was a working spray bottle with a blue liquid being stored on a shelf in the server area with no common name on the container. PIC said it was glass cleaner. COS Chemical was marked.
- KDA_7_20111B - --There was a working spray bottle of glass cleaner, Clorox cleaner, and toilet bowl cleaner being stored on a shelf directly above several packages of pizza boxes located in the back kitchen area. COS Chemical was moved.
