COS-pans and lids were moved to three compartment sink for washing. KDA_6_50111 - --The following area's had stained ceiling tile: four in the women's restroom, four in the pop storage room and one in the dining area. KDA_6_50112A - --The dining area, has an accumulation of dust on the ceiling fans and the ceiling tile. KDA_6_50118 - --The hand washing sink in the kitchen has an accumulation of dirt and grease. KDA_2_30114H - --The cook cracked raw shell eggs into a bowl, then added milk. He then took his gloves off and put on new ones. He then touched two pieces of ready to eat sliced cheese. Educated employee on hand washing. COS-hands were washed, and cheese slices were discarded. KDA_3_30212 - --Sitting beside the hand washing sink in the cooking area, there was a shaker bottle with a yellowish liquid in it with out a common name. Person in charge said it was vinegar for fruit flies. On a shelf under the pop machine on the left, there was a container with a white substance in it with out a common name. Person in charge said it was pretzel salt. In the back room on a shelf, was a container with a dull white powder in it with out a common name. Person in charge said it was instant potatoes. COS-these items were marked with common name. KDA_3_30412B - --In a cabinet under the popcorn machine, there were single serve cups stored in the popcorn kernels. They were without handles. COS-cups were removed. KDA_3_50114D - --In the reach in make table on the west side, raw shell eggs were 61.3F. COS-eggs were discarded. KDA_3_50116A2 - --In the make table on the west wall: cooked ground beef crumbles 61.4F, cooked sausage crumbles 60.1F, shredded cheese 58.5F, pizza sauce 58.2F. Ambient was 58.6F. COS-those items were discarded. Make table on east wall: ready to east deli turkey 47.6F. Employee said it had been in there since yesterday. COS-turkey was discarded. KDA_3_50118A2 - --In the walk in cooler, there was a ready to eat half ham with out date marking. Employee said it was opened yesterday. In the reach in make table on the east wall an opened half gallon of milk was with out a date mark. Employee said it was opened on Saturday, there was also a bag of opened lettuce with out a date. Person in charge said it was opened on Sunday. In the first drawer under the grill, ready to eat deli sliced turkey was without a date. Employee said it was opened on Friday. In the same drawer ready to eat deli ham slices were without a date. Employee said it was opened on Sunday. COS-items were dated with correct dates. In the first drawer under grill, there was ready to eat deli ham slices with a date of 9/20. Person in charge said that was the date it was put in the freezer. Educated on date marking when putting in freezer. COS- dated with the date it was taken out minus the day it was out before freezing. In the 2nd drawer under the grill, there were hot dogs with out a date. COS-hot dogs were discarded. KDA_4_204112B - --The make table on the west wall is without a thermometer. KDA_4_60111A - --In the back room, there were three ladles stored hanging on a rack with food debris on them. Above the toaster, stored as clean was a knife with dried on food debris on the handle. COS-items were moved to three compartment sink. KDA_4_60111C - --The fronts of the make table on the west wall have dried on food debris. KDA_4_60211E4 - --The ice machine in the back room has black on the metal deflector and behind the deflector. KDA_4_90311A - --In the front area, sitting on the floor were two sleeves of Styrofoam cups in plastic wrap. Under the hand washing sink in the front area, there were plastic straws and lids stored under the plumbing. Person in charge said they use those items. COS-items were moved to a shelf. KDA_6_30111 - --In the back room, the hand washing sink did not have soap in the dispenser. COS-Dispenser was filled. KDA_6_50111 - --In the women's restroom there are four stained ceiling tile. There are four stained ceiling tiles in the pop storage room. there is one stained tile in the dining area. KDA_6_50112A - --Under the soup warmer, there is an accumulation of dirt and debris. The ceiling fans in the dining area have an accumulation of dust. There are several ceiling tiles in the dining area with an accumulation of dust. KDA_7_10211 - --Under the hand washing sink in the back, there was a spray bottle with a clear liquid. Person in charge said it was vinegar. COS-bottle was labeled.
