- 4-302.14 - --The QT test strips that were in use for the 3 vat sink and the wiping cloth buckets expired 6/30/2012. COS- PIC voluntarily discarded and found another set of test strips that are not expired.
- KDA_3_30211A1b - --There were containers of raw hamburger stored in the top portion of make table 2 on the cook line directly above a container of RTE chicken patties and a tray of RTE sliced cheese. There was no divider provided between the top and bottom of the make table. COS- PIC moved the chicken and cheese to the walk in cooler.
- KDA_3_30212 - --There was a container holding an unidentifiable product on a shelf near the warewashing/prep area without a common name listed on the label. PIC identified the product as seasoning.
- KDA_3_50116A1 - --There was chili that measured at 106F in the crockpot on the line. PIC could not identify when the item was prepared. COS- PIC voluntarily discarded the chili.
- KDA_4_20211A2 - --There was a rubber spatula on the clean utensil rack with a broken and cracked food contact surface. COS- PIC voluntarily discarded.
- KDA_4_60111A - --There were approximately 10-15 plastic food storage containers with old date sticker residue along the sides stored on the clean utensil rack in the warewashing area. COS- PIC moved the containers back to the 3 vat sink for additional cleaning.
- KDA_4_90311B - --There were several plastic food storage containers stacked together wet on the clean utensil rack not allowed to properly air dry.
- KDA_6_501110B - --There were employee jackets hanging on the dry storage rack in the back directly over single service cups.
- KDA_6_50112A - --There was dirt and grime build up along the inside of the cabinets in the lobby area.
- KDA_7_20111A - --There was a wiping cloth bucket of QT and a working spray bottle of surface cleaner stored on a shelf under the front counter directly next to and touching several bags of single service straws. COS- PIC moved the chemicals to a different area.
- KDA_Defacto - --cheese 40 (make table 1 on line) hot dogs 38 (reach in cooler 1 on line) raw hamburger 38 (make table 2 on line) chicken 39 (reach in cooler 2 on line) onions 37 (cold hold on line) shredded lettuce 42 (make table 3 on line) sauce 35 (reach in cooler 3 on line) sliced tomatoes 41 (reach in cooler 4) chili 39 (walk in cooler) milk 40 (reach in cooler front) --Chlorine measured at 50ppm in dishmachine, test strips were provided. --hamburger 170 (grill) --hot dog 155 (hot hold unit) cheese 136 (dispenser) fries 139 (hot lamp) --QT measured at 200ppm in wiping bucket on cook line, in prep area and at front counter. --There is a pest control company contracted, no paperwork provided. No activity. --There was an opened bag of RTE hot dogs in reach in cooler 1 on the line without any date marking. PIC stated the item was opened today 1/6/16.

- 4-302.14 - A test kit or other DEVICE that accurately Measures the concentration IN mg/L of SANITIZING solutions shall be provided.
- KDA_3_30212 - Food storage containers (Common Name) Except for containers holding FOOD that can be readily and unmistakably recognized such as dry pasta, working containers holding FOOD or FOOD ingredients that are removed from their original packages for use in the FOOD ESTABLISHMENT, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be identified with the common name of the FOOD.
