  • KDA_3_10111 - --In the RIC in the kitchen there were packages opened containers of deli turkey, roast beef, and bologna that were not date marked. PIC said she didn't know when they were opened. COS, VD.
  • KDA_3_30211A1b - --In the WIC a box of raw bacon was stored on a box of RTE beer. COS, moved.
  • KDA_3_30212 - --A bottle of clear liquid on the sandwich prep table did not have a label on it. The cook said it was water. COS, labeled.
  • KDA_3_30511A1 - --In the WIF a box of buns was stored directly on the floor.
  • KDA_3_40311D - --At 12:20 PM,in the 3 compartment hot holding unit a compartment containing white queso dip had a temperature of 128 F. The PIC said she pulled it from the RIC where it had been placed after cooling from the previous day and put it in the hot holding unit at 9:15 AM. COS, VD. Educated staff about proper reheating procedures.
  • KDA_3_50116A2 - --On the prep table a container of sliced tomatoes had a temperature of 57 F. The tomatoes were not held under temperature control. COS, put in RIC. In the ice bath at the sandwich prep station cut lettuce had a temperature of 50 F. COS, moved to RIC. In the WIC a tube of raw ground beef had a temperature of 46 F. At 1:00 PM the PIC said the order arrived at 9:30 AM this morning and the other foods were put in the WIC at 10:45 AM. COS, all PHF's moved to the WIF.
  • KDA_3_50118A2 - --In the RIC an opened package of hot dogs did not have a date mark on them. PIC said they were opened on 11/13. COS, dated.
  • KDA_4_10111A - --In the kitchen potatoes are stored in a non-food grade container. COS, moved.
  • KDA_4_30111 - --The WIC is not holding PHF's to 41 F or below. Ambient temperature 47.3 F
  • KDA_4_60111A - --The fry cutter has dried food debris on it. The PIC said she cleaned it 3 weeks ago. COS, moved to be and cleaned and sanitized.
  • KDA_4_60111C - --There is a sticky residue on containers storing packaged food throughout the kitchen.
  • KDA_4_90311A - --There is a box of single service containers stored directly on the floor in the kitchen.
  • KDA_4_90411A - --On the bar by the lemonade dispenser single service straws are stored in a cup and not wrapped to prevent contact with the lip-contact surface.
