No specific allowed use in restaurants was seen on label at time of inspection. COS-Manager discarded chemical in dumpster,
  • KDA_7_20613A - --On north east side of kitchen, tracking powder between food production table and wall. No substances were present that would show a clean line along wall in a white powder. COS-employee cleaned up powder.

Facility was treated by Orkin pest control Wichita Kansas invoice #105569777 on 10/23/2015. Discussed with manager about keeping pest operator company (Orkin) servicing store after infestation is stopped to help prevent future infestations.

  • KDA_3_50116A1 - --In kitchen, under heat lamp fried egg rolls 124.6 and sweet and sour chicken 123.7. COS-Manager chose to discard. VD Facility is using some time as a control procedures but these products are not included. Discussed adding egg rolls and sweet and sour chicken to list.
  • KDA_3_50118A2 - --In reach in refrigerator, on north side of kitchen, sweet and sour chicken not date marked. COS-manager said chicken was made 10/17/2015 so he date marked to correct.

COS-Manager chose to place containers in dish area for cleaning. In kitchen, at food final preparation line, 2 white plates with stuck on rice. COS-Manager chose to move plates to dish area for cleaning.

In kitchen, on back splash to 3 compartment sink 3 young cockroach like bugs found. Manager killed 6 of the bugs found. Manager showed invoice of Orkin pest service treatment of the facility on 9/29/2015. In kitchen, 4 dead cockroach like bugs between mop service sink and walk in cooler, under true air freezer 20+ dead cockroach like bugs. Manager had employees sweep up dead bugs. NONC 10 day follow-up.
