Located in the dish area, there was one personal drinking cups sitting on the shelf above the clean dishes. COS. Cup was moved. Educated staff. Showed PIC a sample of a cup holder that can be hung on the wall for personal drinks. KDA_3_50118A2 - --Located in the make table, there was one open gallon of milk with no open date. PIC said it was opened on October 26. COS. Milk was dated. Educated staff. KDA_4_20211A2 - --Located on the shelving for clean dishes there was one china bowl out of approximately 200 china dishes that had chips on the rim. COS. Bowl was placed in trash can. KDA_4_50111A - --The chest freezer lid is being held together with duct tape and there is an accumulation of dirt debris around the tape. KDA_4_50111B - --The make table in the cooking area has 2 doors that have broken and non-working gaskets. KDA_4_50112 - --The cutting boards on the make tables have an accumulation of black debris in the cut indentions. KDA_4_60111C - --The large white salsa container, located in the WIC, has an accumulation of dried food debris on the lid and exterior. The chest RIF has an accumulation of ice build up and also has dirt debris on the lid and exterior. KDA_4_90311B - --Located at the soda station there were approximately 25 stacked wet drinking glasses. Educated staff. KDA_5_20212A - --The ladies and mens toilet room hand sinks only have a hot water temperature of 68F. The hand sink in the cooking area has a hot water temperature of 92F. The hand sink in the bar area has a hot water temperature of 75. KDA_5_20511B - --The hand sink at the bar has an accumulation of pink sticky residue from staff using the sink to pour out drinks. COS. educated staff. KDA_5_20515B - --The garbage disposal is leaking water under the sink area. The mop sink faucet was leaking water. PIC said the water was turned on and under pressure and that is why it was leaking. COS. Water was turned off and faucet stopped leaking. The hand sprayer in the dish area has a constant drip. KDA_6_20215D - --Upon arrival the back door was propped open approximately 8 inches with a brick. COS. Door was closed. KDA_6_30114 - --There is not any hand washing signage at the hand sink in the bar area. COS. Signage was given to PIC KDA_6_501114A - --The storage room has a large accumulation of clutter and it prevents easy access to the supplies on the shelving. KDA_6_50112A - --The WIF and kitchen walls have an accumulation of dried food debris. The floor area under the soda station, in the storage rooms, and under the cooking equipment have an accumulation of black sticky debris. The floor area under the chip station, in the WIC, and in the dish area have an accumulation of slimy water and wet food debris. This accumulation is from the leaky garbage disposal.
