The bar waitress had a wiping cloth attached to her shorts and she was repeatedly using the cloth to wipe her hands and equipment. These items were VD. diced tomatoes 54.9F KDA_3_50118A3 - --Located in the bar RIC there was one bottle of mixed liquor made with heavy whipping cream dated September 25. COS. Liquid was VD. KDA_4_20211A2 - --Located on the shelf for clean dishes, there was one wooden spoon with splinters and cracks and when the spoon is touched the splinters fall off. COS. Item was placed in trash can. KDA_4_30111 - --The make table #3 had an ambient air temperature of 53.8F, using the lollipop thermometer and waiting 15 minutes to read thermometer. Tomatoes and cheese had been in the unit more than 4 hours and had to be VD. KDA_4_60111A - --The wall mounted French fry cutter had dried black food debris on the food surface. Employees did not know when it was last washed. KDA_4_90311B - --Located at the soda machine station, there were approximately 15 drinking glasses stacked together wet. KDA_6_501114B - --The ground area and premises around the outside of the building have a large accumulation of litter, trash, dead leaves, broken equipment. KDA_6_50112A - --The walls in the kitchen have a large accumulation of dried food debris and grease. The floor of the breaker box room and the hot water tank room have a large accumulation of trash debris, lent, and dead bugs. The outside WIC floor area, has an accumulation of trash debris and dried food debris. The door to the WIC, the door to the kitchen, the doors to the storage rooms all have an accumulation of black sticky debris. KDA_6_50116 - --There were two wet mops not hanging to air dry. KDA_7_10211 - --Located in the hot water tank storage room, there was a bucket of dark liquid with no label. PIC did not know what it was. COS. Bucket was dumped. KDA_3_30512 - --There is a storage room in the mens toilet room that is not secure with a locked door and food items and single use items are stored inside the room. KDA_3_50116A2 - --Located in the make table #3, there was one pan of cooked chicken wings with a temperature of 57F. Ambient air temperature of unit was 65.3F. Employee did not know how long chicken had been in unit. COS. Chicken was VD. KDA_3_50118A3 - --Located in the beer RIC, there was one bottle of house mixed drink with heavy cream as an ingredient. Date on bottle was 9-11-15. COS. Drink was VD. KDA_4_20211A2 - --Located in the dish area on the rack for clean dishes, there were 7 cracked plastic lids. COS. Employee placed in trash can. KDA_4_30111 - --The make table #3 had an ambient air temperature of 65.3F. Chicken being stored more than 4 hours was VD. PIC is trying to replace unit with a newer model which is sitting outside. KDA_4_50111B - --The WIC door is being held together with duct tape and also it has large holes and cracks on the interior side and door is not cleanable. KDA_4_60111A - --The wall mounted French fry cutter had dried food debris. Employees did not know when it was last used or washed. KDA_4_60111C - --The WIC door has a large accumulation of black sticky residue on the exterior and edges. The WIC vent covers have a large accumulation of lent debris. The vent hood filters have a large accumulation of grease residue. The interior of the beer RIC has an accumulation of food an sticky debris in the bottom. KDA_4_60211E4 - --The ice machine guard late has visible black mold on the edges. KDA_4_90311A - --Located under the pool tables, there are cases of napkins and single use trays sitting on the floor. KDA_6_20214 - --The ladies toilet room door is not self-closing. KDA_6_501114B - --The ground area and premises around the outside of the building has a large accumulation of litter, trash and garbage. KDA_6_50112A - --The walls and floors in the kitchen area have a large accumulation of greasy residue. The WIC floor and walls have an accumulation of food debris. The vent covers in the toilet rooms have a large accumulation of lent debris.
