KDA_4_60111A - --There was old date sticker residue along the top of a plastic food storage lid and on the side of a plastic food storage container on the clean utensil rack in the back. COS- PIC moved the items back to the 3 vat for additional cleaning.
KDA_4_70211 - --There was no chemical (Chlorine) reading at the dishmachine in the back while in use by staff. There was extreme corrosion along the tube connection to the Chlorine container in the cabinet. PIC moved the dishes to the 3 vat for additional cleaning and instructed staff to use 3 vat only until dishmachine has had maintenance.

KDA_7_20111B - --There was a dispensing container of hand sanitizer stored on top of the bulk ice unit in the back storage room. COS- PIC moved the chemical to a different area. There was a working spray bottle of Clorox cleaner on the back counter directly above several clean cups in the cabinet below. COS- PIC moved the chemical to a different area.
KDA_Defacto - --milk 38 (front reach in cooler) milk 38 (back reach in cooler) milk 40 (glass reach in cooler by 3 vat) --QT measured at 200ppm in 3 vat, QT test strips were provided. --There were opened gallons of milk in the front reach in cooler and in the back reach in cooler without any date marking. PIC stated the milk products were opened yesterday 12/9 in the evening. COS- PIC correctly date marked.
