- KDA_3_30212 - --Observed on shelf in dry storage area, 2 grey plastic tubs containing what white granular substance with no common name. PIC stated one was salt and one was seasoning for rice. COS- labeled.
- KDA_3_30412D - --Observed rice spoon sitting in standing room temperature water next to rice steamer. PIC stated that is what they use when they scoop rice. COS- explained to PIC the different ways spoon can be stored in order to be in compliance.
- KDA_3_30511A1 - --Located in the dry storage area, 2 bags of rice stored directly on the floor.
- KDA_4_10111A - --Observed 2 plastic sterilite containers of previously fried chicken, 1 sterelite container of previously fried rice, and 2 sterelite containers of previously cooked noodles. Observed large blue plastic toy tub on ground of dry storage area near prep sink storing uncooked rice.
- KDA_4_204112B - --No temperature measuring device observed in prep table.
- KDA_4_60111A - --Observed dried on food debris on blade of counter mounted can opener blade. COS- taken to three vat sink washed/rinsed/sanitized.

- KDA_6_50112A - --In the corner near the prep sink and dry storage area there is rice debris on the floor. There is debris and detritus on the floor in the walk in cooler.
