  • KDA_3_10111 - --There was one case of 30 dozen raw shell eggs sitting on the floor at a temperature of 54.4F. PIC said they came in on Friday, January 22, and he forgot about them. COS. Eggs were VD and taken to dumpster.
  • KDA_3_30211A1a - --Located in the WIC there were 5 packages of raw shell eggs sitting on the shelf above raw ginger root. COS. Eggs were moved to bottom shelving.
  • KDA_3_30511A1 - --PIC is storing up to 3 pallets of dry food items outside on the sidewalk all day during business hours. Food is exposed to splash, dust, wetness, rodents, and other contamination. Foods need to be moved inside the FE or inside a storage shed. There are 5 cases of noodles sitting on the floor. PIC said the food items came in on Friday, January 22 and he is waiting on new shelving that he ordered.
  • KDA_4_10111A - --Located inside the WIC there were 2 "sterlite" non-food containers with fresh raw lotus root immersed in water and the lotus root was touching the container. COS. PIC moved the root to approved plastic bowl.
  • KDA_6_30112 - --Located inside the unisex toilet room, there was not any hand drying device. The dispenser was empty. COS. PIC placed single use towels in the toilet room.
  • KDA_Defacto - --Pest control company is treating facility weekly at this time. --The FE is no longer cutting or packaging food items. The electric slicer has been removed from the FE. This facility does not have any type of ware-washing system. --WIC: raw shell eggs 40.5F

  • KDA_3_10111 - --There are 10 bags of dry rice with holes from rodents eating through the bag and there is fresh rodent droppings around the rice packages. There were approximately 50 single items in the WIC with fresh rodent droppings all over the packages. There were approximately 130 cases of food items in the WIC with fresh rodent droppings on top of all of the cases. There was one case of food items with 2 packages of bacon with mold and 2 bags of mushrooms that were swelled and bubbly. There was one case of gallon jars of kemchi, the jars were all leaking from the top and they were sitting at room temperature and not under refrigeration. PIC said they were there more than 24 hours. All of these items were VD by the owners.
  • KDA_3_20111F - --There is packages of what PIC identified as lamb in the retail freezer with no safe handling instructions.
  • KDA_3_30511A1 - --There are more than 100 cases of food items sitting directly on the floor: noodles, cookies, produce, Rice. There are more than 25 cases of meat sitting on the floor in the WIF.
  • KDA_3_50116A2 - --WIC: bean sprouts 57.4F, raw bacon 57.6F, cut greens 57F, enoki mushrooms 55F, oriental vegetables 55F, bok choy 58.9F, white shimeji mushroom 68.9F, tofu 55.9F . The WIC has been out of order since December 228th per text messages sent between owner and service company that was shown to inspector.
  • KDA_3_60211B1 - --There is packages of what PIC identified as lamb in the retail freezer with no common name label.
  • KDA_3_60211B3 - --There is packages of what PIC identified as lamb in the retail freezer with no net contents on label.
  • KDA_3_60211B4 - --There is packages of what PIC identified as lamb in the retail freezer with no business information on label.
  • KDA_4_30112A - --There is no three compartment sink in the establishment. The establishment uses a meat slicer with no means of sanitation.
  • KDA_4_60111A - --There is a meat slicer that has dried food debris on the blade and food contact surface. PIC stated the slicer was last used 1/10/16. PIC said he uses it to cut meat and cleans it the next day. Educated staff on cleaning. However, there is not a 3 compartment sink in the FE to wash dishes.
  • KDA_4_60111C - --The WIC vent covers have a large accumulation of lent debris.
  • KDA_6_30112 - --There is not any hand drying supplies in the toilet room.
  • KDA_6_501111A - --There are more than 300 fresh rodent droppings in the WIC on top of the food items and all over the floor in the entire FE. There are rice bags with holes that are broken open from rodent activity. There is one case of drinks with a hole in it from rodent activity.
  • KDA_6_501111B3 - --The PIC said they do not use a pest control company and have not been able to keep control of the rodent problem.
