- KDA_2_30114H - --When inspector notified employees of the sanitizer compartment was about to overflow. The employee went to the warewashing sink with gloved hands. He turned off the faucet and return to touch the meat in MT 1. When address the employee, he said he had on 3 single use pairs of gloves and took off a pair and did not have to wash hands. COS Employee was educated, employee took off gloves and washed hands.
- KDA_2_40111 - --Observed an employee chewing gum while working at the front counter putting customers food orders inside brown bags. COS Employee was educated by PIC. Employee took gum out of his mouth and discarded. He also washed his hands and discarded meat.
- KDA_3_30511A1 - --In the storage area, a bag of drink carriers were stored on the floor. COS PIC stored the carriers on a shelf.
- KDA_3_50116A1 - --Chicken tender 130F was being held hot near the deep fry station. PIC said they have been there for 30 minutes. COS PIC drop the chicken tender in the deep fryer for rapid heating.
- KDA_4_60111A - --Stored as clean on a storage rack, stainless steel containers were stacked and had stickers residue on the exterior surface. COS PIC was educated. Containers were removed place in the warewashing area to be scraped , washed, rinsed, and sanitized.
- KDA_4_90111A - --Stored as clean on a storage rack near the warewashing area, were food contact stainless steel containers were stacked wet. COS Containers were removed and placed in the warewashing area to be washed, rinsed, and sanitized.
- KDA_6_501111A - --Observed 15 drain flies (small flies) in the back storage area on the mop bucket and mop. Contracted pest control last visit was 10/21.
- KDA_6_50116 - --A wet mop was stored in mop sink basin to dry. COS PIC hung the mop up to air dry properly.
- KDA_Defacto - --3 vat sink, Quat 400ppm Quat test strips were available.
