KDA_5_20515B - --There is a leak below the left compartment sink at the 3 vat sink when water is drained.
KDA_5_40211 - --There were 9 bags of raw frozen chicken thawing in the 3 compartment sink (below the flood rim) that is directly plumbed. COS-placed bags in the True RIC.
KDA_3_20215 - --In the dry food storage room there were 2 large cans of Liberty Gold Crushed Pineapples with deep dents on the side of the cans. COS-pulled for vendor credit.
KDA_3_50114A2 - --In the True reach in cooler there was a blue plastic food grade container with mashed potatoes made on 10/20/15 at 44.8F. There was condensation on the lid and sealed tightly (not able to cool down properly). The ambient air temperature of the unit was reading 39.7F. COS-discarded.
KDA_3_50115B - --In the True RIC there was a blue plastic food grade container of mashed potatoes with condensation on the lid and the lid was sealed tightly. The product was not able to cool down properly to 41F within 6 hours. COS-discarded.
KDA_4_60111A - --On a shelf to the left of the stove, there was one metal food container with dried food debris and stored clean. The large can opener connected to the counter had oil and food debris build up on the blade (it was used today). On top of a cabinet there was 2 plastic food containers with the foil seal left on the rim (holds bacteria). COS-moved to the ware wash area. In the cabinet there were 2 round plastic lids with the cardboard seal left on the inside and one of the lids had a label left on it and stored clean. COS-discarded.
KDA_4_70211 - --The mechanical dish machine was in use, utilizes chlorine for sanitization and measured at 0ppm. KDA 61 primed the machine twice and still reading 0ppm. COS-set up the 3 compartment sink with bleach and it measured it at 50ppm. (Note: the person in charge called for the unit to be serviced).
KDA_5_40213 - --The mop sink located in the chemical storage room was clogged and sewage water backed up the service sink and spilled over the flood rim onto the floor. The sewage water was contained to the chemical storage room. COS-the person in charge called for it to be serviced. The plumbers came on-site during the inspection. They were able to unclog the drains and cleaned the area to prevent any further contamination.
KDA_6_50111 - --The baseboard strip below the mechanical dish machine is peeling away from the wall.
KDA_7_10211 - --In the 3 compartment sink there were 3 white plastic buckets with clear liquid and not labeled. The cook said one was Power 7 #507, one was bleach water and the other one was soap and water. COS-labeled all 3 buckets.
