Observation: Dried debris build up on the blade area of the large table slicer in the back prep room. Person in charge cleaned and sanitized the slicer.
- Physical facilities maintained in good repair
Observation: Water damaged ceiling tiles observed in the back hallway near the office. Person in charge stated there is a roof leak that needs to be repaired.
- PHF/TCS foods maintained at 41°F or below, except during preparation, cooling, cooling, or when time is used as a public health control
Observation: Chopped lettuce(48F), diced tomato(48F), tzatzki(49F), ambient air(50F) inside the three door line cooler was not able to maintain proper cold holding temperature. Person in charge discarded perishable foods stored in the cooler overnight. A service call was placed with Leets Refrigeration. No perishable foods can be stored in the cooler until temperature is holding 41F or below.
- Hand drying provided
Observation: No paper towels at the hand washing sink in the back prep room. Person in charge replaced the paper towels.

Observation: No Verifiable Employee Reporting Agreement available onsite. Inspector supplied the person in charge with FDA form 1B Reporting Agreement to review and have employees sign.

Observation: Food debris build up on the slicer below the blade. Slicer was not used today. PIC cleaned and sanitized the slicer.

Observation: No light shield covering florescent bulbs above the prep table in the back room.

Observation: Ice build up on the floor of the walk in freezer due to a drip from the condenser unit. PIC will have the ice removed.

Observation: Gap still observed at back establishment door. Entry is not tightly sealed. Repeat violation from last inspection.

Observation: No hand washing reminder sign posted in the employee restrooms.
