Observation: No Employee Illnesses Reporting Agreement. Employee stated that he is aware not to come but do not remember that he signed the form.

Observation: Pressure for sanitizer cycle on dish machine must be maintained 15-25psi. Dish machine service tried to repair yesterday. Bringing in a new tech today.

Observation: Rusted metal shelving in reach-in pizza prep cooler is on order to be replaced. New cutting board also on order.

Observation: 1) dish machine kick plate is loose creating a slight leak when running, repair 2)Urinal water flows over bowl when flushed. Need a deeper bowl.

Observation: Clean vent in restroom.

Observation: 1) Recaulk behind and around hand sink / food prep sink counter in front pizza prep area. 2) Provide cove base for wall/floor juncture behind shelving for large flour bins in back room. 3) Remove duct tape around plumbing clean out under hand sink in back room. Seal if needed. 4) Side wall near dough press is crack and damaged, repair 5) Loose wall on right side for mop sink. Repair. All surfaces must be smooth and easily cleanable.

Observation: Pressure gauge for hot water sanitizer dish machine must be maintain 15-25 psi. Gauge is reading over 30psi. Repair to ensure proper sanitation.
