Observation: Mop is not position too proper drying and was found wet in a mop bucket. If not in immediate use the mop must be allowed to air dry to prevent buildup of bacteria. Corrected on site. COS
- Cleanability of floors, walls, and ceilings
Observation: 1) Lower section of wall is missing around floor in back storage area, missing around floor. All walls must be easily cleanable and a cove base is required at the floor/wall juncture. Make repairs as needed. 2) several tiles are damaged and have holes. Floor coverings, walls, wall coverings, and ceilings must be designed, constructed, and installed so they are smooth and easily cleanable.
- Food is properly labeled
Observation: Self serve pastries and donuts have no labels with ingredient information for sale in a pastry case in the middle of the store. Donuts must be labeled with the following information in plain view of the consumers a list of ingredients in descending order of predominance by weight, including declarations of artificial color or flavor and chemical preservatives, if contained in the donuts.
- Food storage - preventing contamination from the premises
Observation: 1) Boxes of bananas stored behind the cahier on the floor, 2) past shelf due cookies ( banana’s cake)also stored behind the cashier on the floor. Food must be stored at least 6" off the floor and covered to prevent contamination. Manger discarded expired cookies (bananas’ bead) and removed the bananas on the shelf. Corrected on site. COS

Observation: In the walk in cooler two types of hot dogs are being held for 14 days. PHF if not used in 24 hours SHALL BE DISCARDED AFTER 7 days. All employees were educated and verbalized. The date was changed during this inspection when to discard after both were open. CORRECTED ON SITE.
- Handwashing signage
Observation: Both hand washing sinks do not have signs posted when employees shall wash their hands. ALL handwashing sinks used by food employees shall have signs posted at the handwashing sink. Given during this inspection TO POST AT BOTH HANDWASHING SINKS. CORRECTED ON SITE.

- Physical facilities maintained in good repair
Observation: THIS IS A REPEAT VIOLATION FROM SEVERAL INSPECTIONS. The wall leading into the warewashing area has a hole and it appears that this was caused by the door handle (knob). THIS SHALL BE REPAIRED WITHIN 10 DAYS DUE TO REPEAT VIOLATION NUMEROUS INSPECTIONS. Send verification with letter of correction this area was repaired such as invoice or receipt.
- Drying mops
Observation: One mop is not in a position for proper air drying. After use, mops shall be placed in a position that allows them to air-dry without soiling walls, EQUIPMENT, or supplies. CORRECTED ON SITE.
- Posting inspection reports
Observation: The most current inspection is NOT posted. SHALL post this inspection in a conspicuous for consumer to view. --Observation: The most current license for RS and FE is not posted. The only posted licenses have ALL EXPIRED. The manager made a copy of the two licenses from the file to post until the originals can be found. Shall post in a conspicuous place for consumers to view.
- Food is properly labeled
Observation: The self serve pastry case holding various types of cookies and pastries. No igredient labels are available for the consumers to view. During this inspection this inspector education the staff where to get the labels off the box the food items were shipped in. Employees started to place the ingredient labels in a book, continue to have all the food items available for any allergies.
- Hand drying provided
Observation: The back hand washing sink in the warewashing area does not have any paper towels. If using paper towels for hand drying, shall ALWAYS have available for proper hand drying after washing hands. CORRECTED ON SITE.
