Observation: Observed meat department sanitizer buckets in excess concentration. Tested on the sanitizer test paper as over 500 parts per million. This is from a mechanical premix sanitizer equipment.

Observation:Observed dividing trays in raw meat storage case with food build up. Employee stated that these trays get cleaned and sanitized weekly.

Observation: Observed cut melon, mixed fruit, breakfast pizza, salsa, and pineapple in customer reach in produce cooler had a temperature of between 47 and 49 degrees. Observed thawed frozen pizzas on top of the customer reach in freezer area. Items removed and discarded. COS
- Package integrity
Observation: Observed two packages of frozen tater tots and one package of frozen onion rings rewrapped with plastic wrap around existing package in the customer reach in freezer. COS by removing.
- Outer openings are protected
Observation: Observed a half inch gap below the closed loading dock doors.

Observation: Observed machine oil in an unmarked spray bottle in meat department. Bottle was labeled. COS
