Observation: No verifiable Employee Health Reporting Agreement available onsite at time of complaint investigation. Person in charge was able to verbally explain the establishments sick policy. No written procedures were available onsite. Inspector provided person in charge a copy of the FDA 1B Employee Reporting Agreement. Manager stated the document will be signed by each staff member and placed in the employee files.
