Observation: Condensation buildup was found in the walk in freezer. Condensation does not come from a potable source and therefore is a contaminant. Condensation must be investigated and repaired if possible.

Observation: The shelves opposite the dish machine and service counter shelves are both peeling paint and must be resurfaced. Also cardboard was observed being reused, and paper towels used for moisture retention. Cardboard can only be used for original product and paper towels may not be used for moisture retention.
- Storage of clean linens, single-service, and single use articles
Observation: Single use items were stored on the floor in the area by the furnace. Must store all food/food items at least 6 inches off the floor.
- Cleanability of floors, walls, and ceilings
Observation: Entrance to the walk in freezer (broken) and the area outside the freezer (missing) have floor surfaces that are no longer smooth, easily cleanable, and non absorbent.
- Storage and maintenance of wet and dry wiping cloths
Observation: Wet wiping cloth was found on top of a sanitizer bottle. Wet wiping cloths must be maintained in sanitizer to prevent buildup of bacteria.
- Sanitizers
Observation: Sanitizer was tested too high at time of inspection. Sanitizer was remade during inspection. Must maintain quat sanitizer between 200-400ppm or at manufacturers specification.
- Test kit provided and used to measure sanitizing solution concentration
Observation: Test strips were expired at time of inspection. Must purchase test strips within expiration date.
- Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
Observation: The following areas have become soiled and must be cleaned and or resurfaced: Metal racks in the walk in freezer (rusty), wire racks @ stairs (dusty), and hot wells in buffet table (soiled).
- Equipment location, installation, repair, and adjustment
Observation: Delfield pizza cooler (by drive-thru) and the 4 door prep cooler (line) both have torn door gaskets on all doors. Gaskets must be replaced.
- Heating, ventilating, and air conditioning systems design, installation, and cleaning
Observation: Vent covers in the walk in freezer have become dusty, and the area above the hoods have grease buildup. Must clean.

Observation: Cooked chicken in reach-in pizza preparation cooler was date marked 4/15/14. Must discard cooked chicken after 7 days from date mark. Corrected on site.

Observation:Observed raw eggs stored above diced packaged potatoes and other ready-to-eat foods. Must store raw eggs below any ready-to-eat foods to prevent potential cross-contamination.

Observation: 1)Clean build in both microwaves near kitchen grill. 2)Slight growth on front ice chute inside large ice machine. Clean and sanitize more frequently to prevent build-up and possible contamination. 3) Defrost reach-in freezer near fryers.

Observation: 1)Degrease inside convection oven. 2) Clean food debris on stove/oven unit used for roasts. Clean as often as necessary to prevent build-up.

Must keep food items at least 6 inches off floor for food protection and easy cleaning. REPEAT VIOLATION. 2) Ice build-up in walk-in freezer. Drain line from condenser may be leaking. Remove ice from food area. and repair if needed.

Bag off eggs stored above vegetable in walk-in cooler. Must store on bottom shelf below ready-to-eat foods to prevent cross-contamination. Corrected with education.

1) Door tracks of reach-in ice cream freezer, 2) In between cutting boards in back of kitchen where chopper and meat slicer is stored, 3) Door cracks and crevices of all reach-in coolers, and 4) Top of dishmachine all have food build-up. 5) Fan near meat slocer has heavy dust build-up. Clean more frequently to prevent build-up and dust contamination. 6) Kickplate/barrier for condenser to the Cold Tech reach-in freezer is missing. heavy dust on intake grate. Clean to ensure unit maintains to work properly and prevent dust contaminating food.
- Sanitization of food contact surfaces - before use and after cleaning
Dishmachine was not dispensing sanitizer. Bucket was empty. Switched to a new bucket of sanitizer. Corrected on site.
- Roasts held at a temperature of 130°F or above
Delfield pizza reach-in cooler was holding pizza sauce at 44 degrees F. Turned cooler down .Holding 41 degrees F at end of inspection. Watch thermometer incase dial is moved and cooler out of temp. Corrected on site.
