- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation: No thermometer in under counter cooler (juice drink station). Person In Charge placed thermometer in cooler.

Observation: Sanitizer (chlorine) solution concentration too low. Demonstrated with Person In Charge and employee how to make solution at proper concentration.

Observation: Food (open topping containers) stored below paper towel dispenser.

- Plumbing system maintained in good repair
Observation:The 3 compartment sink plumbing leaks water into a bucket and is in need of repair. The owner stated that they have contacted a plumber for repairs. Was not required to correct this violation by today's date.
- Duties of PIC
Observation:Observed customer walk into the establishment from outdoors, walk behind the customer service counter and begin preparing himself and serving himself coffee. REPEAT VIOLATION. A policy must be created addressing this violation to ensure this practice is no longer allowed. The policy and the Letter of Correction must be submitted to this Department by April 7, 2014.
- Food is properly labeled
Observation: Prepackaged ice cream cakes prepared onsite for customer self-service are labeled only with the name of product. Establishment owner stated that they are in the process of ordering these labels from the franchise. Establishment was not required to correct this violation by today's date.

(B) Label information shall include: (1) The common name of the FOOD, or absent a common name, an adequately descriptive identity statement

Observation: Observed container of raw chicken breasts stored above tortillas in the kitchen prep cooler. The chicken was moved to the lowest shelf with the other raw meats. Corrected at time of inspection.

Observation: Condiments on display not protected from customer contamination-containers of pickles and onions on the counter for customer self service are not covered or other wise protected.

Observation:Paper towels are not provided at the handsink located by the soft serve dispensing machines. The paper towel dispenser is located about 4 feet away directly above food dispensing containers.

Observation: Observed cheese grater stored in a container of cheese with the handle in contact with the food product.
- Duties of PIC
Observation: Observed person who was not working at the Dairy Queen (an employee of an adjoining business) walk into the food prep area and fill a cup for himself at the fountain pop machine, talked with the PIC in this food prep area, and then walk out of the business.

Observation: Food handler placed shredded lettuce on a taco with her bare hands for service to a customer.

Observation: Prepackaged ice cream cakes displayed for customer self-service do not have labels listing the names of major food allergens contained in the FOOD items(dairy, tree nuts, etc.).
