    Observation: Machine/ equipment that is not in use now, has the lard not covered and have no lids. All food must be protected from contamination. Plastic wrap were placed on the food item in the kitchen CORRECTED. COS
  • Hand drying provided
    Observation: Observation: 1) Paper towel dispensers was empty at hand sink in the kitchen. 2) Paper towel dispenser was empty in men's restroom. Must provide disposable towels at every handwashing sink. Restock now. CORRECTED ON SITE

    Observation: Employee preparing food with no hair restrain. She stated that she just came in to the shift.All employees must wear effective hair restraints to prevent contamination. Corrected on site.

    Observation: All working containers with salt, papers and other seasoning were not labeled on the food counter.Must label all working container with correct common name for easy identification. used marker and corrected on site.

    Observation: Has our forms for Clean up procedure but no Clean- up kit and their written procedure .Food establishment must have procedure for employees to follow when responding to vomiting and diarrheal events that involve the discharge of bodily fluids such as vomitus or fecal matter onto any surface within the food establishment to reduce foodborne disease transmission.     Observation: No handwashing sign on hand sink in ice machine room. Every hand sink to be used by employee must has sign posted reminding employees to wash their hands.
  • Restricted use pesticide applied by a certified applicator
    Observation: QA sanitizer in spray bottle was tested over 400ppm. QA sanitizer concentration must be maintained 200-400ppm. Must use test kit to ensure solution concentration after each made.
  • PHF/TCS foods maintained at 41°F or below, except during preparation, cooling, cooling, or when time is used as a public health control
    Observation: Sandwich prep table on left side is out of temperature, 46*F. Food in this cooler was temped between 43.8*F-47.3*F. Discarded all potentially hazardous food and removed others. Cooler temperature must be maintained 35*F or low to cold hold food internal temperature at 41*F or less. MAY NOT USE COOLER UNTIL APPROVAL HAS BEEN GIVEN THROUGH HEALTH DEPARTMENT. Called for service. REPEAT VIOLATION

Must replace gasket to ensure the cooler temperature can be maintained properly. 2) The first door on reach-in freezer had ice buildup, and the self-closing device is broken. Must repair the self-closing device and the door can close tight. 3) The rack on dressing side in sandwich prep cooler is rusting, and no longer cleanable. Replace to prevent contamination. 4) Observer four damaged rubber scraper. Discarded.
