Observation: Wooded divider in chest freezer and reusing cardboard boxes for storage. Surfaces exposed to food must be smooth, nonabsorbent, and easily cleanable. Suggest plexi glass in freezer and discontinue reusing boxes.
- Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
Observation: Open bag of pepperoni and chicken wings in reach in cooler are not dated. When opened, prepped, or cooked food must be dated and than discarded within 7 days. Educate staff.
- Handwashing signage
Observation: Every hand sink to be used by employees must have sign posted reminding them to wash their hands. Post signs at hand sinks.
- Food storage - preventing contamination from the premises
Observation: Cups in the kitchen and parm and pizza sauce in dry storage were on the floor. All food and food items must be stored a minimum of 6" off the floor. Move to shelves
- Storage and maintenance of wet and dry wiping cloths
Observation: Wiping cloths must be stored in sanitizer in between uses.
- Sanitizers
Observation: Using a disinfectant to clean tables. Must use an approved sanitizer to wipe down tables. Suggest using kitchen sanitizer.

Observation: Must post most recent inspection in area that the public can readily see it.

Observation: Must post license in an area readily viewed by the public.

Observation: Paper towel dispenser at hand sink by ice machine is empty. Every hand sink must provide a means to dry hands. Restock paper towel.

Observation: 1) Hand sink in between ice cream chest and pizza prep table requires splash guards on both side to prevent contamination from splashing. 2) Shelving unit in dry storage has bottom shelf installed 2" off the floor. All food and food items must be stored a minimum of 6" off the floor. Move shelf.
- Outer openings are protected
Observation: Door to mini golf is propped open. Light is shining through the bottom of back doors. All outer openings must be protected from the entrance of bugs or rodents. Keep door shut or install a tight fitting screen door. Apply weather stripping to back doors.
- Cleanability of floors, walls, and ceilings
Observation: 1) Caulk each hand sink in kitchen and 3 tank sink. 2) Wall patch under 3 tank sink must be smooth, nonabsorbent, and easily cleanable. Repair. 3) Install FRP around mop sink. 4) Flooring in dry storage must be easily cleanable and sealed with commercial kitchen approved product. Provide this dept specs on intended product. 5)Install cove base in dry storage area. 6) Gas line to pizza oven is on the floor. raise up to aid in cleaning.

Observation: Raw eggs being stored over drinks in the reach-in cooler in dry storage. All food must be stored in manner to prevent contamination. Moved to bottom shelve. CORRECTED.
