Violation descriptions and comments

Assorted herbs in the walk in cooler were being stored in raw meat boxes being reused for food storage. Do not reuse containers or boxes for food storage. Items were removed, boxes were thrown out, and herbs were washed and stored properly.
The thermometer in the walk in cooler was not functioning properly.
The cutting boards in the dough preparation room were stained.
The drainpipes under the three compartment sink and the food preparation sink were soiled with grime. The ventilation hood filters were soiled with dust and grease. The door to the walk in freezer was soiled with food debris. Several bulk containers used for food storage throughout the kitchen were soiled with food residue.
Several utensils were stored with their food contact surface exposed in a bucket on the storage rack near the office. Several plastic food containers on the top shelf of the storage rack near the three compartment sink were stored upright with their food contact surface exposed.
The caulking on the handsinks in the mens and womens restroom was torn. The handsink to the right of the food preparation sink leaked onto the floor. The mopsink was soiled with grime.
The floor edges along the Hobart ovens and under the metal preparation tables were soiled with food debris. The floor drains in the dough preparation room were soiled with debris. A ceiling tile above the walk in cooler was missing. Two large holes were present in the wall below the preparation room restroom handsink. The floor underneath the food preparation sink and the three compartment sink were soiled with food residue. The floor area in the dough preparation room under the tables was soiled with food debris.
